feat(sandbox): surface structured mount upload result on E2B sandbox (#4884)
* feat(e2b-sandbox): make mount upload deadline configurable Replace the hardcoded 120-second mount upload deadline with a configurable `mount_upload_deadline_seconds` key read from SandboxConfig (extra=allow). The value is validated: zero and negative inputs are clamped to 1 second. Omitting the key preserves the existing 120-second default. This addresses the follow-up from PR #4842 review: operators with large mounts or slow networks can now size the deadline to their deployment without changing code. * fix(e2b-sandbox): address review feedback on configurable deadline - Remove import-time default capture from _mount_deadline_reason() and _MountUploadBudget.deadline_seconds to prevent silent drift. - Add warning log when mount_upload_deadline_seconds is clamped to 1 (was silent before). - Update AGENTS.md E2B Mount Uploads section: deadline is now configurable, not fixed 120. - Add mount_upload_deadline_seconds to YAML examples in provider docstring and __init__.py. - Add config-path test that exercises SandboxConfig -> _load_config -> _apply_mounts end-to-end. * feat(e2b-sandbox): surface structured mount upload result on sandbox Introduce MountUploadResult dataclass and attach it to E2BSandbox.mount_upload_result after creation. This makes mount truncation observable in code without re-parsing Gateway logs. _apply_mounts() now returns MountUploadResult with truncated, reason, and upload totals. _create_sandbox() captures the result, stores it on the sandbox instance, and records it in a provider-level map so the result survives warm-pool reclaim and reconnect. MountUploadResult.truncated is True only when the upload pass was stopped early by a resource limit (deadline, file count cap, or byte budget). Individual mount failures (missing host path, SDK errors) are logged but do NOT set truncated. Tests cover: success totals, deadline truncation, file-count truncation, byte-budget truncation, non-limit failure not reported as truncation, missing host path not reported as truncation, create→sandbox wiring, and create→release→warm-pool→acquire result preservation. * fix(e2b-sandbox-provider): fix _mount_results lifecycle leak and review findings - Add _forget_mount_result() helper and call it at all terminal sandbox paths: _reuse_in_process_sandbox dead-evict, _reclaim_warm_pool_sandbox reconnect/dead/bootstrap/ownership/shutdown failure branches, _forget_local_sandbox, _kill_and_close. Prevents unbounded dict growth over a long-running Gateway process. - Make MountUploadResult @dataclass(frozen=True) to prevent silent mutation of the shared reference between provider map and sandbox attribute. - Move _mount_results insert under self._lock in _create_sandbox to match the read discipline in _register_connected_sandbox. - Guard _resolve_mount_upload_deadline against None (YAML explicit null) to avoid int(None) TypeError. - Add 5 regression tests covering each bypass path and the frozen invariant. * fix(e2b-sandbox-provider): add _forget_mount_result to _evict_oldest_warm branches Add _forget_mount_result() calls to all four terminal exit paths in the E2B _evict_oldest_warm override (reconnect failure, already-gone, kill failure, kill success). feature(gateway): issue request trace ids unconditionally (#5119)
* refactor(gateway): issue request trace ids unconditionally The request trace id was gated behind logging.enhance.enabled at every entry point, so downstream code had to keep asking whether one existed: a header-provenance flag in its own ContextVar, a precedence resolver, and three-level carrier fallbacks at each consumer. Bind one unconditionally instead. TraceMiddleware covers Gateway HTTP; ensure_trace_context covers the entry points that never touch ASGI -- scheduled occurrences, MCP task notification runs, IM channel messages, and the embedded client -- each scoped to one unit of work so a long-lived worker task cannot leak one occurrence's id into the next. The ContextVar becomes the only source; the response header, runtime context, run metadata and log records are derived outputs. Consumers now use ensure_trace_id() or resolve_trace_id(*carriers) and drop their presence guards. Removed: resolve_deerflow_trace_id, the header-provenance flag and its three helpers, set/reset_current_trace_id, is_trace_correlation_enabled and its gateway alias. BREAKING CHANGE: every Gateway HTTP response now carries X-Trace-Id and it cannot be turned off; logging.enhance.enabled controls log output only. Installations on the default enabled: false will start seeing the header. No config keys were added or removed. * fix(gateway): stop persisting a caller-supplied trace id on the run record body.metadata forks two ways: through build_run_config into the live run config, which the run worker restamps, and through create_or_reject into the run record that the runs API echoes verbatim. Only the first was covered, so a client sending metadata.deerflow_trace_id made the most durable and most visible surface of a run disagree with the X-Trace-Id and the log lines the same request produced -- a correlation id that does not match the logs is worse than none. Stamp the server-issued id once at the trust boundary so both forks receive it, preserving the caller's own metadata keys. Close the same gap on config.context, which reaches the runtime context by a separate path: _build_runtime_context no longer merges server-owned keys from the caller, and _install_runtime_context assigns rather than setdefaults. A thread's metadata is no longer seeded with the run-scoped id of whichever run created it -- one thread spans many runs and as many trace ids. Found by driving a real run through the Gateway and reading the run back from the runs API; every unit test built its metadata by hand and so could not see it. * fix(gateway): expose X-Trace-Id to split-origin browser clients X-Trace-Id is not on the CORS safelist, so a browser client served from a separate origin could not read it -- and those are exactly the clients that cannot read the Gateway's logs either, leaving them with nothing to quote in a bug report. Same-origin nginx deployments were unaffected, which is why this stayed hidden. Add it to CORS_EXPOSED_HEADERS beside Content-Location, referencing TRACE_ID_HEADER rather than repeating the literal. * fix(gateway): keep X-Trace-Id on unhandled-exception 500s Starlette's ServerErrorMiddleware sits outside every user middleware and emits unhandled-exception 500s through the raw send, so those responses never pass TraceMiddleware's header-writing wrapper. The 500 for a server bug is exactly the response a user most needs to correlate with a log line, and it was the one response that shipped without the id. TraceMiddleware now tracks whether http.response.start has been sent. On an exception with no response started it emits its own plain 500 carrying the header, then re-raises: the outer ServerErrorMiddleware sees the response already started and only re-raises too, so the server's exception logging is untouched. An exception mid-stream keeps propagating unchanged — a second response start cannot be sent, and the already-written header stands. The trace id is printable ASCII by construction (normalize_trace_id / generate_trace_id), which is what makes the raw latin-1 header encoding safe. * fix(gateway): strip the forged trace id from the persisted request echo The run-record fix stopped a forged metadata.deerflow_trace_id on the authoritative metadata surface, but the raw request echo still carried one: create_or_reject persists body.config verbatim as runs.kwargs_json, which the runs API serves back. A client posting config.context.deerflow_trace_id therefore still got its forged value stored and echoed on one API surface while the header, logs, run metadata, and checkpoint all carried the real id — the id is ignored as input there, so echoing it back only manufactures disagreement. Two changes close it. redact_config_secrets — already the shared scrub for that echo, applied at admission and again at serve time, so historical records are covered too — now also drops deerflow_trace_id from config.metadata and config.context. And build_run_config now merges run metadata onto a copy of the caller's config["metadata"] instead of updating it in place: the nested values of the request config are reference copies, so the in-place merge was writing the server-stamped key through into body.config, contaminating the "what the client sent" record before it was persisted (and incidentally masking the forged-value echo on the metadata container). The regression test posts a forged id through body.metadata, config.metadata, and config.context at once and reads the kwargs echo back off the run record, failing if either leak returns. * docs(harness): record the trace-echo scrub, 500 fallback, and accepted retry divergence The trace section of the harness AGENTS.md now covers the two fixes that close the derived-output rule (the kwargs-echo scrub in redact_config_secrets plus build_run_config's copy merge, and TraceMiddleware's own 500 for unhandled exceptions), and CHANGELOG gains their Fixed entries. It also writes down the one accepted divergence: a crash-recovered scheduled launch reuses the durable run through its idempotency key, and start_run returns early on idempotency_reused without restamping — so the run record keeps the first attempt's deerflow_trace_id while the retry's own log lines carry the freshly minted id of its ensure_trace_context binding. The divergence is confined to the crash-recovery window and is accepted rather than fixed: restamping on reuse would rewrite a persisted record for a run that already exists, which is worse than two ids that each correlate their own attempt's logs. Written down so the next reader of the scheduler recovery path does not diagnose it as a bug. * docs(config): align the logging.enhance schema note with the unconditional trace id The config-module AGENTS.md still described logging.enhance as the gate for the Gateway X-Trace-Id header and Langfuse deerflow_trace_id. That model is gone: ids are issued unconditionally and this block decides log output only. Left as-is, the stale wording invites an agent to "restore" a header gate it believes was lost. Reworded to match the sibling AGENTS.md files and config.example.yaml, with a pointer to the Request Trace Context section that owns the full model. * docs(changelog): link the trace entries to #5119 The five new entries pointed at the ([#XXXX]) placeholder with no reference definition, rendering as literal text instead of a link — and RELEASING.md step 2 relies on those references when the section becomes release notes. All five now point at #5119, with the definition appended to the reference block. * refactor(harness): rename _stream_without_trace_context to _stream_turn The name asserted the opposite of what the method now does. It was accurate while logging.enhance.enabled could route stream() around the trace scope; with the gate gone it is the only stream implementation left, and it binds the id itself via ensure_trace_id(). Private, so the rename touches only the definition and the one stream() call site. * docs(harness): fit the trace-context guidance inside the AGENTS.md chain budget The expanded Request Trace Context section pushed the effective AGENTS.md chain for agents/middlewares to 99,815 bytes, past the 98,304 hard limit scripts/check_agent_guidance.py enforces in CI (AG002). Compressed the section from 7,359 to 4592 bytes with no facts removed: the entry-point table, the derived-output rule and its enforcement points, the accepted scheduled-retry divergence, the two resolution helpers, the stream() binding rationale, the log-output-only gate, the CORS listing, the 500 fallback, and the test map all remain. Sized against the merge, not just the branch: current main grew the same chain by ~724 bytes, so the check was verified on the merged tree as well (97,772 bytes; branch tree 97,048). * fix(gateway): declare content-length on the fallback 500 The pre-response 500 declared content-type but no content-length, leaving the framing to the ASGI server: chunked on HTTP/1.1, close-delimited on HTTP/1.0 — the one wire difference from the ServerErrorMiddleware response it replaces, which sends content-length: 21. The explicit header keeps the fallback byte-identical to what clients saw before. * docs(readme): drop the trace-correlation condition from the translations The zh/ja/fr/ru Langfuse sections still said metadata.deerflow_trace_id matches X-Trace-Id "when request trace correlation is enabled". The id now always matches and that condition no longer exists, so each bullet states the unconditional match and that logging.enhance.enabled only controls whether the id is printed into logs — the one piece of the feature a user can still configure. * test(gateway): pin TraceMiddleware wiring through create_app() Every X-Trace-Id test exercised a hand-built four-route app, so the real stack's add_middleware(TraceMiddleware) line was pinned by nothing: deleting it — or short-circuiting above it — passed CI while silently dropping both the response header and the ambient id the run-record stamp and enhanced log records derive from. One case now drives /health through create_app() and asserts the inbound id round-trips; mutation-checked by removing the wiring line, which fails exactly this test. * docs(gateway): note the fallback 500 is CORS-opaque The pre-response 500 is emitted outside CORSMiddleware — the exception has already unwound past it — so it carries no Access-Control-Allow-Origin and a split-origin browser client cannot read the id on this one response, unchanged from the ServerErrorMiddleware 500 it replaces. Documented on the class and in the CHANGELOG entry rather than fixed: replicating the origin allowlist outside CORSMiddleware would let the two policies drift. * fix(harness): keep abandoned-stream cleanup inside the trace binding stream() binds the turn's id around each next(inner) and resets it before yielding, but the finally's inner.close() ran after that binding was gone. Abandoning the stream therefore drove the inner LangGraph generator's GeneratorExit/finally path with no trace id — or an unrelated ambient one from whichever context ran the close — so cancellation and finalization logs and callbacks did not correlate with the turn they belong to. inner.close() is now wrapped in a local bind/reset of the same turn id. The token is set and reset in the same frame, never across a yield, so the per-step cross-context safety is preserved even when GC closes the generator from another Context — pinned by the existing copy_context close test, which now exercises this path. The regression test records the id from the inner generator's finally and fails without the binding. * test(harness): teach the worker-trace fake about RunManager.cleanup Upstream #5112 (bound gateway memory after terminal runs) added a run_manager.cleanup(run_id) call to run_agent's finalization, so the merge-commit CI run failed all five worker-trace-binding tests with AttributeError on this PR's _FakeRunManager. The fake gains the same no-op shape as its other methods. * docs(gateway): bring the gateway AGENTS.md back under its soft budget Upstream #5092 grew backend/app/gateway/AGENTS.md to 40,966 bytes, 6 over the 40,960 soft budget that test_agent_guidance_check.py::test_repository_guidance_stays_below_soft_budgets_and_avoids_doc_indexes enforces — its Unit Tests run on main was cancelled by push concurrency, so main is currently red on that test and every PR merge-run inherits the failure. * fix(messages): drop legacy <uploaded_files> tag handling (#4212) PR #4174 unified upload-context injection on <current_uploads> (IM and web both flow through UploadsMiddleware), and #4632 documented the current path. This removes the remaining backward-compat parsing of the pre-#4174 <uploaded_files> tag, the final cleanup item tracked by the issue: - deermem: only <current_uploads> is stripped from human turns before memory persistence, and the upload-sentence scrubber drops the legacy tag alternative. - mem0: the mirrored message filter recognises only <current_uploads>. - InputSanitizationMiddleware: remove the legacy tag from the blocked-tag denylist (it existed only because deermem parsed the old tag). - frontend: stripUploadedFilesTag / stripInternalMarkers / parseUploadedFiles and the message-list fallback parse only <current_uploads>; demo thread fixtures are migrated to the current tag. Scope decision: a <uploaded_files> block in pre-#4174 history is now treated as ordinary user content (pinned by tests in both layers) instead of being silently dropped or stripped. * style: apply prettier formatting to stripUploadedFilesTag * fix(uploads): keep legacy <uploaded_files> stripping for display/export only Addresses review feedback on #4826: removing the legacy tag from the frontend display layer made pre-#4174 threads render raw <uploaded_files> XML (with server-side upload paths) in chat, copy data, and JSON exports. The backend cleanup stands — memory pipelines and the sanitization denylist treat only <current_uploads> as an internal marker. The frontend keeps the legacy spelling in its display/export-only utilities (stripUploadedFilesTag / INTERNAL_MARKER_TAGS / parseUploadedFiles and the message-list fallback) so old history renders cleanly without leaking internal paths, while the memory/sanitization scope-decision tests remain unchanged. Frontend tests now pin both spellings: <current_uploads> and legacy <uploaded_files> are stripped from copy data, markdown leak-stripping, and JSON exports. * docs(ui): record accepted display-spoof tradeoff for legacy upload tag Review note (willem-bd): since <uploaded_files> is off the sanitization denylist, a live user can type the legacy spelling and fabricate file chips / hide their own message text in display. Display-only and self-inflicted with no backend semantics, so it is accepted for now; documented at both the message-list fallback and stripUploadedFilesTag. The Honcho backend landed in #4730 without user-facing docs: the main README's Long-Term Memory section covers the other opt-in backends (mem0, openviking) but never mentions honcho, and unlike mem0 the backend shipped no guide README. - Add backends/honcho/README.md mirroring the mem0 guide structure: configuration (with the plain-HTTP api_key guard), workspace-per-user isolation and fail-closed identity, recall/search behavior per mode, limitations (no fact CRUD -> gateway 501, no DeerMem migration), and async/failure-policy semantics. - Add a short honcho paragraph + guide link to the README Long-Term Memory section, alongside the existing mem0 paragraph. |

Add _stable_id() (sanitize_id output + 8-hex-char SHA-256 suffix of the raw id) and use it on the default (non-override) path in _workspace/_user_peer; workspace_overrides / user_peer_overrides still match on the raw key, unchanged. * feat(extensions): observe task lifecycle and system model calls PR 1 (#4636) gave extensions a middleware chain, and a middleware only sees what passes through the agent graph. Two runtime surfaces stay invisible to it: when a lead run or a subagent begins and ends, and the DeerFlow-owned model calls made outside the graph. This slice adds both, with no new Gateway surface -- routers, services, and the reference extension stay in PR 3. Contract (deerflow-extension-api 0.1.1) --------------------------------------- Two contribution kinds join `middlewares` on the registry: `task_lifecycle` (`on_task_start` / `on_task_stop`, receiving a `TaskInfo` and a conservative `TaskOutcome` of completed / aborted / failed) and `system_model_observer` (`on_system_model_call`, receiving a `SystemOperationKind`, a `SystemModelRequest` snapshot, and a `SystemModelResult` carrying either the response or the provider exception plus a duration). `SystemModelRequest.messages` normalizes to a tuple at construction. Goal evaluation and memory extraction pass a message list while title generation and summarization pass one prompt string, and a bare `str` already satisfies `Sequence` -- without normalization an observer iterating `request.messages` would silently walk characters. Copying also makes the frozen snapshot immutable in fact rather than only by declaration, since observations may run after the call site returns and keeps mutating its own list. Registry marks and rollbacks become per-bucket and positional, so an `install()` that fails after registering two different kinds cannot leave one of them behind. `needs_task_store` now covers all three kinds: a deployment that registers only lifecycle hooks still gets a task store. Task lifecycle -------------- The lead worker notifies start after the run has started and stop after completion persistence and the completion hook, but before clearing the finalizing barrier and publishing the stream end -- holding the barrier across stop is what keeps a same-thread replacement run from overlapping this task's lifecycle. Cancellation raised out of the stop notification is deferred, not propagated in place, so a cancelled run still clears the barrier and emits its end frame. A subagent with a parent `run_id` wraps its execution in the same pair inside `finally`, reporting `parent_task_id` so a delegation tree is reconstructable; a subagent without a `run_id` (embedded client, standalone LangGraph Server) logs and skips rather than inventing a parent. Contributors run in registration order inside one shared 3s budget and every failure is logged and failed open. System model calls ------------------ Four kinds cover the model calls the middleware chain cannot see: goal evaluation, memory extraction, title generation, and summarization. Each site reports both terminal paths without changing the provider exception the host observes, short-circuits on `has_system_model_observers`, and passes the live task store when the runtime has one (detached work gets an isolated store). The sync summarization half stays unobserved on purpose -- it and its only host caller are the sync side of an async-only runtime, so notifying there would block a thread on a call site the host never reaches; the reason is recorded at the call site. The DeerMem backend must stay vendorable and cannot import the extension API, so it reports through a new `MemoryCallbacks.on_memory_llm_result` host hook that the DeerFlow-side callbacks translate into an observation. Notification loop ----------------- Extension resources must be touched on the loop that created them, but subagents can execute on isolated loops and DeerMem runs on a worker thread. The Gateway registers its serving loop before any runtime dependency starts and resets it last through the exit stack, so every startup-failure and cancellation path is covered. Awaited hooks raised on another loop are dispatched across with `run_coroutine_threadsafe` and awaited under the same budget; synchronous sites submit fire-and-forget work. Shutdown stops accepting detached observations before the memory flush -- that flush runs on a worker thread and can emit memory observations -- while keeping the loop alive for awaited task hooks until run and subagent drain completes. Tests ----- `test_extension_task_lifecycle.py`, `test_extension_subagent_lifecycle.py`, and `test_extension_system_model_calls.py` cover ordering, fail-open, budget exhaustion, snapshot binding under a concurrent singleton replacement, the loop-dispatch and shutdown-suspension paths, and both terminal paths at every call site. `test_gateway_run_drain_shutdown.py` pins the stop-before-barrier and drain ordering. * fix(extensions): decide notification fail-open by origin, observe cancellation `_notify_each` only guarded `Exception`, so a contributor letting a `CancelledError` escape — an extension implementing an internal timeout with cancellation, say — skipped its successors and reached the worker's deferred-interrupt path, ending an otherwise successful run as cancelled. Fail-open is about where a failure came from, not its base class: only a genuine cancellation of the host task increments `Task.cancelling()`, so propagate on that and contain everything else. `KeyboardInterrupt` / `SystemExit` still propagate. `observe_system_model_call` skipped observers on cancellation for the same base-class reason, leaving goal / title / summarization silent on a terminal path that is routine — interrupt/rollback admission and shutdown both cancel the run task, with the provider tokens already spent. Awaiting observers there is unreliable (a repeated cancel interrupts that await before any of them runs), so report through the same non-blocking submission the synchronous memory bridge uses, then propagate the cancellation untouched. DeerMem keeps `BaseException` around its provider call, now with the reason recorded: that path runs on a worker thread, where cancelling the awaiting side never interrupts the running thread, so `CancelledError` cannot arrive at all. Its host-hook wrapper narrows to `Exception` — only the hook's own failures are non-fatal, and an observability path must not swallow a process teardown signal. * fix(extensions): warn on budget exhaustion, scope observer logs by task, propagate teardown Review response on #4684: - The memory observation bridge caught BaseException, which would swallow a teardown signal raised while dispatching; it now catches Exception, matching the boundary the DeerMem-side call site documents and tests. - A notification-budget timeout raised mid-hook fell into the generic hook-failure path and logged an asyncio-internal traceback; it now logs a warning like the pre-hook budget skip, while a TimeoutError a contributor raises on its own stays classified as a hook failure. - System model observer logs passed the operation kind as the task id, so log lines said "task goal/title/..."; they now carry the task scope id alongside the kind. |

memory_add's duplicate check ran outside the storage critical section
(the tools.py comment called this out): two concurrent tool calls for
the same user could both pass the check and both store the same fact.
Move authoritative duplicate rejection into
MemoryUpdater.create_memory_fact: the candidate's normalized content
key is checked against the fresh memory snapshot on every
revision-conflict retry, so the loser of a concurrent create reloads,
sees the winner's fact, and is rejected with ValueError("Duplicate
fact"). The tool-layer pre-check stays as a fast path.
The REST router now maps the duplicate ValueError to 409 with a clear
detail instead of the misleading "content cannot be empty" 400.
Tests: backend-level duplicate rejection, a simulated
concurrent-commit-during-conflict-retry race, and the router 409
mapping.
* fix(memory): retry legacy-path fact creation on save conflict
Wrap the legacy single-file save in the same bounded conflict-retry
loop as the apply_changes path: reload the fresh snapshot and re-run
_raise_if_duplicate_fact_content on every retry, so a concurrent
duplicate commit is rejected with ValueError("Duplicate fact") (tool
error / REST 409) instead of the generic OSError save failure
(REST 500). No-duplicate conflicts now retry and store instead of
failing the create.
Addresses review feedback on bytedance/deer-flow#4599.

* feat(memory): signals-based update pipeline + always-on watermark/trivial filter
Refactor the DeerMem memory update pipeline (message_processing -> queue ->
updater) around a signals frozenset seam, replacing the
(filtered, correction_detected, reinforcement_detected) 3-tuple with
(filtered, signals: frozenset[str]) end to end.
message_processing:
- Externalize signal-detection patterns to YAML (message_patterns/*.yaml).
- Extend signals from correction/reinforcement to a 6-class set
(correction/reinforcement/preference/identity/goal/decision); detect_signals
returns a frozenset aligned with the fact category enum.
- Pure-acknowledgment turns ("ok"/"好的"/...) are always filtered out before
enqueue (whole-message fullmatch), saving an extraction LLM call.
queue (core/queue.py):
- In-memory list + debounce timer, with flush_sync (graceful-shutdown drain
that joins an in-flight worker under a hard timeout) and queue_max_depth
backpressure (signal-bearing updates always admitted; QueueFull otherwise).
- Same-key updates coalesce with a signal union; per-batch success/fail summary.
updater (core/updater.py):
- head500+tail500 message truncation (replaces the 1000-char head chop).
- Always-on per-thread watermark: feed only messages added since the last
extraction. The watermark is in-memory and is not advanced on failure, so a
failed/lost update is re-fed on the next conversation turn.
- [MANUAL] prompt marker for user-authored facts (source.type="manual").
- Post-invoke extraction_callback (host-injected) emitting facts_extracted /
facts_accepted / rejected_low_confidence; the host default logs metrics and
flags >60% rejection.
Confidence filtering remains in _apply_updates (the existing
fact_confidence_threshold check); there is no separate write gate.
Consolidation stays opt-in (lossy). The ABC add/add_nowait signature is
unchanged, so the summarization flush hook and host are unaffected.
Tests: add test_message_processing_signals, test_updater_truncation,
test_updater_watermark; update queue/updater/consolidation/staleness/pluggable
tests for the signals seam.

self-guard; MemoryMiddleware was the lone exception). Emergency
(add_nowait) always admits under backpressure -- its data cannot be
re-fed next turn.
- Rewrite the watermark from index-based to content/identity-based
(_message_identity + _feed_after_watermark) so it stays correct when
summarization removes the conversation front -- an index watermark
pointed at the wrong message and silently skipped un-extracted tail
turns. The emergency flush bypasses the watermark (bypass_watermark on
ConversationContext, threaded through update_memory) and coexists with
(does not replace) a pending normal update, so a flush cannot drop a
pending update's un-extracted tail.
- Populate facts_accepted / rejected_low_confidence inside _apply_updates
at the real confidence-filter site (passed_threshold) instead of
re-deriving the threshold in _finalize_update -- eliminates metric drift.
- Emit extraction metrics in a finally with an 'attempted' flag so
exception failures (parse error, apply_changes raise after retry) are
observable, not only the happy path.
- Re-detect signals on the post-watermark feed for the extraction hint so
it no longer references turns the LLM cannot see; admission-time signals
still drive backpressure.
- Move the post-batch reschedule inside the queue lock to close a
non-atomic self._timer race with a concurrent add.

confidence-gate signal, not a persisted-fact count); drop the stale
"historical semantics" justification. Brand-new callback, one consumer.

onboard with less code and the contract stays stable as capabilities are
added. A minimal backend now implements only from_config + add + get_context
(verified by test_memory_manager_interface.py::_MinimalBackend onboarding via
the factory); the factory no longer knows a backend's private hooks.
- MemoryManager: ABC -> pydantic BaseModel; three-tier methods (tier-1
add/get_context abstract; tier-2 management defaults; tier-3 optional hooks
warm/reload/fact + on_pre_compress/on_turn_start). Dropped 3 self-serving
hooks. 6 hasattr probe sites -> direct call + try/except NotImplementedError.
- from_config classmethod: factory thins to resolve + inject storage_path +
collect host hooks + call from_config; DeerMem-specific hook consumption
moved from factory to DeerMem.from_config.
- Invariants: @model_validator (mode='tool' requires search via supports_search
ClassVar); DeerMemConfig storage_path-is-file check moved here from factory.
- Async: aadd/aget_context/asearch default to the sync path (speculative).
- Callbacks: MemoryCallbacks + LangfuseMemoryCallbacks; on_memory_llm_call
subsumes tracing_callback (same signature/timing/mutation); deleted the
tracing_callback field. DeerMem decoupled from langfuse (portability).
- noop keeps read-op empty overrides (avoids router 500s on the
disable-memory-via-noop path); only delete/export inherit the base raise.
Behavior preserved: 661 passed / 13 skipped. Docs: backends/README.md rewritten
(three-tier + from_config + callbacks); samples README updated; removed stale
private doc paths.

- Read/manage endpoints (GET /memory, /memory/export, /memory/status,
DELETE /memory, POST /memory/import) and the /memory/reload fallback now
catch NotImplementedError -> 501, matching the fact-CRUD endpoints. The
hasattr->try/except migration had skipped these: they were @abstractmethod
before (every backend implemented them, so they never raised), so once they
became tier-2 default-raise a minimal backend (only add + get_context) hit a
raw 500 -- there is no global NotImplementedError handler. get_memory is
shared via _get_memory_or_501 (covers /memory, export, status, reload
fallback). noop is unchanged: its read-op empty overrides never raise.
- warm() base default returns None (tri-state: True=warmed, False=failed,
None=nothing to warm) so the Gateway lifespan logs "skipping" for a
non-DeerMem backend (e.g. noop) instead of the inaccurate "warmed
successfully" it never earned. DeerMem.warm keeps True/False.
- Tests: 6 router 501 tests (read/manage + reload fallback) + 2 lifespan
warm-log tests (None->skipping, False->warning); conformance/pluggable
assert warm() is None.
705 passed / 13 skipped; lint clean.

noop sets supports_search=True to match its search() override. Conformance
adds drift + consistent-backend tests.

("implements neither reload_memory nor get_memory") instead of an uncaught
propagation -- mirrors the router's 501. Test added.

custom backend that onboards without overriding search(). Reworded to drop the
misleading noop example and state both shipping backends implement search().

_extract_text_from_content misses are now also sanitized.
Non-distinguishable paths (< 2 text blocks, non-list content) still
degrade to full sanitization (safe — server block may be escaped but
user forgery never leaks). All fallback paths log via logger.warning.

fix(memory): consolidated facts inherit expected_valid_days from sources (#4225)
* fix(memory): consolidated facts inherit expected_valid_days from sources Consolidation (#3996) and per-fact expected_valid_days (#4143) were both authored by the same contributor but never connected: the consolidated new_fact carried the newest source's createdAt but no expected_valid_days, so _effective_fact_staleness_age fell back to the global staleness_age_days. A merge of several 200-day-old stable facts (each evd=3650) would land with no evd, read as a 90-day window, and re-enter the staleness candidate set on the very next cycle - the merge discarded the lifetime signal of the underlying information and contradicted consolidation's premise (these are stable, related facts worth synthesising). Fix: the merged fact inherits expected_valid_days set so it is re-reviewed at the EARLIEST source review deadline (min(createdAt + expected_valid_days) across sources, relative to the merged fact's createdAt = the newest source's). A merge combines details from every source, so a volatile sub-detail (evd=7) must not inherit a stable source's 3650-day window and escape staleness review for years - staleness KEEP/REMOVE is the only path that re-validates a merged fact, so biasing toward the soonest deadline keeps uncertain merges re-checked sooner. A source already past its deadline yields a minimal positive window (review next cycle) rather than the global fallback, which would defer an overdue review. Capped at the creation-time staleness_max_lifetime_multiplier like any new fact. Omitted when no source carries a valid evd (legacy facts fall back to the global age at read time, matching pre-feature behaviour). DRY: extract _read_expected_valid_days(fact) -> int | None, the shared type rule (int/float, reject bool, coerce to int BEFORE the > 0 guard) previously inlined in four places - _normalize_memory_update_fact, _effective_fact_staleness_age, the newFact creation cap, and consolidation inheritance. All four call the single helper. Coercing before the guard matters for values in (0, 1): 0.5 passes a raw > 0 check but truncates to 0, which would violate the helper's "positive int or None" contract; the order now matches the original _normalize_memory_update_fact rule. No prompt/schema change: consolidation's prompt does not surface source evd to the LLM, so asking the model to assign a merged lifetime would be guessing without signal. Source inheritance is deterministic and always available. Tests: consolidation evd cases now use time-stable createdAt (relative to now via a _days_ago helper) covering - earliest-deadline selection, creation-cap clamp, omit when no source evd, volatile source governs the deadline (and re-enters staleness next cycle), overdue source clamps to a minimal window, float coercion. Plus TestReadExpectedValidDays / TestEffectiveFactStalenessAge regression cases for the (0, 1) coercion-order fix. * fix(memory): reject non-finite expected_valid_days before int coercion The shared _read_expected_valid_days helper (introduced when consolidating the evd type rule across four call sites) coerces with int(raw) before the > 0 guard - reversing the original _normalize_memory_update_fact order so that a fractional 0.5 does not leak as 0. But int(raw) raises for non-finite floats: int(nan) raises ValueError and int(inf)/int(-inf) raise OverflowError. Python's JSON decoder accepts NaN / Infinity as floats by default, so a single malformed expected_valid_days in a hand-edited memory.json would abort staleness selection or consolidation instead of falling back to the global lifetime. On main, _effective_fact_staleness_age checked raw > 0 first, so NaN fell back safely (nan > 0 is false) - but inf did NOT (inf > 0 is true, so main also crashed on inf). This helper is now the persisted-fact read path for both staleness and consolidation, so the regression (and the pre-existing inf crash) must be closed here. Fix: require math.isfinite(float(raw)) before the int() coercion, then keep the existing positivity check and fallback. NaN / +/-inf all return None, so callers fall back to the global staleness_age_days. Normal int/float values (including large ones) are unaffected - isfinite is a no-op for them. Tests: - TestReadExpectedValidDays.test_rejects_non_finite_values - NaN, inf, -inf return None (not raise). - TestEffectiveFactStalenessAge.test_falls_back_for_non_finite_values - the persisted-fact read path returns the global age for each, no raise. - test_consolidation_with_non_finite_source_evd_does_not_raise - end-to-end: a NaN-evd source merged with a stable source does not abort consolidation; the NaN source's effective lifetime falls back to the global 90 and its deadline participates in the earliest-deadline computation. * test(memory): hoist _select_stale_candidates import + tidy deadline docstring Two review nits from the latest pass: - `_select_stale_candidates` was imported inline inside three test methods; hoisted to the module-level import block so the dependency is declared once. - `test_consolidated_evd_volatile_source_with_equal_created_at_future_deadline` had an abandoned calculation in its comment ("3 + 7 = 10 ... minus 3 already elapsed = 7? No:") that could mislead future readers into thinking elapsed-since-creation factors into the inherited window. Collapsed to a single clear line stating the window is relative to the merged createdAt, regardless of the source's current age. * fix(memory): reject huge-int expected_valid_days above timedelta.max.days _read_expected_valid_days routed every numeric value through float(raw) for the math.isfinite guard, but Python's JSON decoder parses an integer literal with no decimal point as an arbitrary-precision int (unlike 1e400, which decodes to float inf). So a hand-edited memory.json carrying "expected_valid_days": 10**400 raised OverflowError in float(raw) before math.isfinite was ever called - exactly the malformed-field-aborts-everything scenario the helper's docstring claims to prevent. The earlier non-finite fix only closed the float cases (NaN / +/-inf / 1e400). A huge int below the float limit but above timedelta.max.days (e.g. 10**12) would pass the helper and raise OverflowError downstream in timedelta(days=evd) during staleness selection or consolidation - the same crash fancyboi999 flagged for extend_by_days, just reached via a stored evd. Fix: branch on type so an int never passes through float() (matching the reviewer's suggestion), AND cap the returned int at timedelta.max.days (999999999) so the downstream timedelta(days=evd) call cannot overflow either. The float branch keeps the isfinite + int() coercion. Both branches share the 0 < evd <= timedelta.max.days positivity/range check. Normal values are unaffected - any legitimate expected_valid_days is far below the cap (the config ceiling staleness_max_extension_days tops out at 36500). Tests (all three layers): - TestReadExpectedValidDays.test_rejects_huge_int_above_timedelta_max - 10**400, 10**12, 10**9, timedelta.max.days+1 return None; timedelta.max.days itself is accepted. - TestEffectiveFactStalenessAge.test_falls_back_for_huge_int_above_timedelta_max - the persisted-fact read path returns the global age, no raise. - test_consolidation_with_huge_int_source_evd_does_not_raise[1e400|1e12|1e9] - parametrized end-to-end: a huge-int-evd source merged with a stable source does not abort consolidation; the bad source falls back to the global 90. * fix(memory): guard datetime arithmetic, not just timedelta construction The huge-int fix capped _read_expected_valid_days at timedelta.max.days, but that only proves timedelta(days=evd) can be constructed - adding it to a real fact timestamp still overflows datetime.max. @fancyboi999 reproduced it: a source with expected_valid_days=timedelta.max.days raises "OverflowError: date value out of range" at dt + timedelta(...) in the new consolidation deadline calculation. capping at timedelta.max.days was another patch chasing the next overflow boundary, not a real close. Root cause: the helper was doing datetime-range validation, but the safe bound depends on the datetime the evd is added to, not on the evd alone. So the responsibility moves to the arithmetic site, with try/except as the terminal guard - no concrete upper bound to be wrong about. Changes: - _read_expected_valid_days returns any positive int (huge ints included, not routed through float). Its job is type/positivity validation only. - New _safe_add_days(dt, days) -> datetime | None wraps dt + timedelta(days), returning None on OverflowError/ValueError. This is the terminal guard - there is no further boundary to overflow because try/except catches any datetime-range failure regardless of magnitude. - _select_stale_candidates uses _safe_add_days(now, -effective_age); a None result means the window is unrepresentably large, so the fact cannot yet be stale and is skipped (not selected). - Consolidation computes each source's deadline via _safe_add_days; a source whose deadline overflows falls back to the global staleness_age_days deadline (same treatment as a legacy no-evd source), so one malformed field cannot abort the merge. Normal values are unaffected - any legitimate expected_valid_days is far below the overflow boundary (the config ceiling staleness_max_extension_days tops out at 36500). Re-ports this feature onto the pluggable-memory backend introduced in #4122 (the original #4143 was force-pushed clean by accident and auto-closed). The #4122 refactor moved the staleness logic into the self-contained DeerMem backend (backends/deermem/deermem/core/) and reverted it to the pre-feature global-threshold version, so the per-fact lifetime work is re-applied here against the new module layout + DI (MemoryUpdater is now (config, storage, llm)-injected; config lives on DeerMemConfig, not host MemoryConfig). **expected_valid_days (creation)** The LLM assigns a per-fact review window when storing each new fact. The prompt exposes five tiers (<=14 d transient -> >365 d very stable). The value is capped at write time by staleness_age_days x staleness_max_lifetime_multiplier (default 20.0 -> 1800 d ~= 5 years; range 1.0-100.0) so the model cannot set an initial lifetime so long the fact is never re-evaluated. The default 20.0 makes the "> 365 d very stable" tier achievable out of the box (3.0 silently clamped it to 270 d). **staleFactsToExtend (review)** During staleness review the LLM can emit extension entries for kept facts whose window seems miscalibrated. new_evd = min(days_since_created + extend_by_days, staleness_max_extension_days). Extensions use an absolute ceiling (default 3650 d ~= 10 years; range 90-36500) rather than the creation multiplier - they are deliberate review decisions that must be able to advance the window beyond the initial cap, but the absolute bound prevents timedelta overflow (a model-supplied extend_by_days of 10**9 previously crashed every later candidate-selection pass with OverflowError) and LLM misfire. **Invariant correctness** - Read-time cap removed from _effective_fact_staleness_age; cap is write-time only so extensions actually advance the review window. - proposed_remove_ids hoisted out of the removals sub-block and used to exclude from extension, so a cap-surviving proposed-removal fact is never extended. - extend_by coerced to int before the > 0 guard (a fractional 0.9 would pass the float check then int() to 0, silently writing a zero-delta extension). - days_since uses total_seconds() // 86400 (not .days truncation). - staleness-section html.escape uses quote=False to match the prompt.py convention; only <, >, & break element-text structure. **Tests** test_memory_staleness_review.py was module-level skipped by #4122 ("full unit-test migration is a follow-up"). This PR performs that migration: DI construction via (DeerMemConfig, _FakeStorage), _build_staleness_section back to the (candidates, config) signature, plus new coverage for per-fact selection, EXTEND with the absolute cap, the overflow next-cycle regression, the proposed-removal-not-extendable case, fractional extend_by skipping, and the creation-time cap. 67 tests, all green. |

fix(memory): treat explicit null backend_config values as omitted in DeerMemConfig (#4217)
config.example.yaml ships backend_config.model: as a bare key whose children
are all comments, which YAML parses to None (make config-upgrade then writes
an explicit model: null). DeerMemConfig.model is a non-Optional field with a
default, so from_backend_config(**{"model": None}) raised a ValidationError
and every run failed with "Input should be a valid dictionary or instance of
DeerMemModelConfig". Drop None entries in from_backend_config so YAML null /
empty keys fall back to field defaults, matching the documented "empty =
host default LLM" semantics. Upstream bug (#4122 schema); regression-pinned
in test_deermem_self_contained.py.

* fix(memory): bounded shutdown flush via MemoryManager.shutdown_flush Re-applies the memory-queue shutdown drain on top of the pluggable MemoryManager abstraction (#4122): the old top-level MemoryUpdateQueue singleton is gone, so the drain is now a backend contract instead of host code reaching into the queue. - MemoryManager ABC: shutdown_flush(timeout) -> bool. Every backend implements a bounded graceful-shutdown drain. - DeerMem: queue.flush_sync (daemon-thread + Event.wait hard timeout for the uninterruptible sync LLM call; joins an in-flight worker first so contexts a debounce Timer already pulled out are not lost on exit; skips inter-item sleep on the drain path; per-item succeeded/failed count), exposed via shutdown_flush. - noop: shutdown_flush is a clean no-op success. - Gateway lifespan: call get_memory_manager().shutdown_flush(timeout) after channels/scheduler stop, via asyncio.to_thread, try/except bounded. No host-level pending/processing guard -- the backend short-circuits on an idle buffer, so the host cannot "forget" the in-flight case (structurally eliminates the guard race flagged on the prior revision). - shutdown_flush_timeout_seconds added to the shared MemoryConfig (host-owned lifecycle budget, default 30, 1-300) + exposed on MemoryConfigResponse and the embedded client; config_version 25 -> 26. Tests: queue flush_sync (7), lifespan drain incl. fix(security): html-escape the conversation block in MEMORY_UPDATE_PROMPT (#4162)
format_conversation_for_update embeds raw user turns into the <conversation> slot of MEMORY_UPDATE_PROMPT. This is the most attacker-influenced input in the prompt, and it was unescaped: a message containing "</conversation><current_memory>..." closes the conversation block and forges a <current_memory> authority section for the extraction LLM, which can be steered into persisting an arbitrary high-confidence fact — and that fact is later injected into the lead-agent system prompt's <memory> block, which the prompt declares trusted. This is the last unguarded sibling of a rule the repo has established repeatedly. #4044/#4060 html-escaped the current_memory slot of this exact template; #4097 escaped the <memory> injection renderer. In updater.py the same .format() call escapes current_memory and leaves conversation raw. The memory updater sees raw text because InputSanitizationMiddleware only rewrites the ModelRequest and never mutates state, while MemoryMiddleware queues the raw state messages. Escape content with html.escape(quote=False), mirroring _escape_summary / _format_fact_line — after truncation so a trailing "..." cannot split an entity, on both human and assistant turns. Render-time only: no stored value is mutated, so the apply path is unaffected. The conversation function already strips <uploaded_files> here, so tag hygiene in this renderer is established. Scope is the memory updater. The summarizer's <new_messages> / <existing_summary> blocks are the same rule unguarded, but their output is quarantined as untrusted durable context rather than promoted to system authority; that hardening will be a separate change. |

used fact.get("confidence", 0.0/0), which only defaults when the key is
absent. A fact whose confidence is explicitly null (or otherwise malformed)
returned None, breaking the numeric sort/format. Use
_coerce_source_confidence, which normalizes null/malformed values and clamps
to [0, 1], so null-confidence facts no longer block staleness handling.

When _process_queue found another worker already processing, it called _schedule_timer(0), spawning a fresh Timer thread immediately and looping tightly (spawn -> busy -> reschedule -> spawn) until the active worker finished. Replace this with a _reprocess_pending flag: a concurrent caller sets the flag and returns, and the active worker reschedules exactly once in its finally block when work remains. Reset the flag in clear(). |

The lead-agent system prompt declares the <memory> block user-managed and
everything else framework-internal, but the injection renderer _format_fact_line
formats a fact's content, category and correction sourceError raw. Memory is
user-editable via /api/memory, so a fact whose content is
'</memory></system-reminder>...' closes the block and relocates the text after
it out of the user-managed trust zone.
Escape those three fields at render time, mirroring the MEMORY_UPDATE_PROMPT
escaping added for the update-prompt side in #4028/#4060. The fact dict is not
mutated, so stored memory keeps the raw value and the apply path is unaffected.
* fix(memory): stop entity-encoding quotes in injected fact text
The three html.escape() calls in _format_fact_line used the default
quote=True, which also converts " to " and ' to '. These fields
are rendered as element text inside the <memory> block, never inside an
attribute value, so escaping quotes buys no defense here: only <, >, and &
can break out of the surrounding tags, and those are escaped either way.
Ordinary facts ("User's preference", 'Said "use Python"') reached the model
as User's preference / Said "use Python" -- content the
lead-agent prompt declares as user-managed data the model should discuss
freely. Pass quote=False and extend the benign-content test, which used a
string with no quotes and so never exercised this path.
Note the escape in updater.py's consolidation_candidates block renders into
an XML attribute value and correctly keeps quote=True.

Fix UnboundLocalError in memory injection when facts are empty (#3992)
`format_memory_for_injection` bound `facts_header` / `all_fact_lines` only inside the `if isinstance(facts_data, list) and facts_data:` block, but the structure-aware overflow-truncation path at the end of the function references both unconditionally. When a user's memory has sizeable user-context / history (so `sections` is non-empty and the assembled output exceeds `max_tokens`) but an empty or missing `facts` list, that block is skipped, so the truncation branch hits `UnboundLocalError: cannot access local variable 'all_fact_lines'` and aborts memory injection entirely. Hoist the two initializers to function scope, alongside the existing `guaranteed_line_tokens = 0`, so they are always bound. Behaviour is unchanged when facts are present. Adds a regression test (empty facts + oversized user context) that fails with UnboundLocalError before the fix and truncates gracefully after. |

Fix KeyError in staleness review when a fact has no id (#3993)
The apply-time staleness guardrail built ``candidate_ids`` with a direct
``f["id"]`` access over ``_select_stale_candidates`` output:
candidate_ids = {f["id"] for f in _select_stale_candidates(current_memory, config)}
Every other fact access in ``updater.py`` uses ``f.get("id")``; this was the
lone direct-subscript outlier. An aged, non-protected fact that lacks an
``id`` key — common in legacy / hand-edited / migrated ``memory.json`` — is a
valid staleness candidate, so it reached ``f["id"]`` and raised
``KeyError: 'id'``, aborting the entire background memory-update cycle for
that user. The guardrail runs unconditionally (independent of the
``staleness_review_enabled`` flag), so any id-less aged fact triggers it as
soon as the LLM returns a non-empty ``staleFactsToRemove``.
Skip id-less candidates when building the intersection set. They can never
be targeted by the id-based removal set anyway, so behaviour is otherwise
unchanged.
Adds a regression test with an aged, id-less fact that raises KeyError
before the fix and applies cleanly after.

* feat(memory): add memory consolidation to synthesize fragmented facts When a fact category accumulates many individual entries, the LLM reviews them during the normal memory-update call (same invocation, no extra API cost) and decides whether groups of related facts can be synthesized into a single richer fact. This completes the memory lifecycle: extraction → guaranteed injection → staleness review → consolidation. - Select fragmented categories by min-facts threshold, surface the most fragmented groups first; prompt-layer caps aligned with apply-layer guardrails so the LLM never sees groups it cannot act on - Cap consolidated confidence at source maximum to prevent inflation; reject results below fact_confidence_threshold - Double-consume protection prevents a fact from being merged into multiple consolidation targets - Feature-gated at both prompt and apply time with per-cycle safety caps - Add 26 tests covering candidate selection, normalization, apply guardrails, and prompt integration * fix(memory): address consolidation correctness issues from PR review Six fixes based on maintainer review of #3996: 1. Deduplicate sourceIds in normalization — ["f1","f1"] previously bypassed the ≥2-distinct-sources check; dict.fromkeys collapses it to ["f1"] which is correctly rejected. 2. Run consolidation after max_facts trim — previously, sources were deleted then the merged fact could be evicted by the trim, leaving no record of either. Moving consolidation last ensures source facts exist in the post-trim index before removal. 3. Fix count= attribute in consolidation prompt — advertised the full category size but listed only max_sources IDs; now uses min(len(group), max_sources) to match what the LLM can act on. 4. Exempt staleness_protected_categories from consolidation candidates — mirrors the existing staleness-review contract so correction facts are never surfaced for merging. 5. Strip and default category in consolidation normalization — " " or " preference " are now normalised, matching _normalize_memory_update_fact. 6. Propagate sourceError from source facts into consolidated fact — correction context is no longer silently lost on merge. * fix(memory): add apply-time guardrails and tests for consolidation P1: mirror the staleness-pass defense-in-depth pattern — build allowed_source_ids from _select_consolidation_candidates at apply time so a protected-category or below-threshold fact proposed by the LLM is rejected regardless of model behavior. P2a: test that LLM-returned confidence is capped at max source confidence and that a capped result below fact_confidence_threshold is rejected. P2b: test that factsToConsolidate with consolidation_enabled=False is a no-op at apply time (35 tests, all pass). * fix(memory): address three correctness issues from second review round 1. Default consolidation_enabled=False — consolidation is lossy (source content is permanently replaced, only consolidatedFrom IDs preserved); new lossy features default to off. config.example.yaml updated to match. 2. Unify confidence coercion between prompt and apply — _build_consolidation_section now calls _coerce_source_confidence(fact) instead of an inline 0.0-default coercion, so a null-confidence fact renders with 0.50 in the LLM prompt and is capped at 0.50 at apply time (same value, same function). 3. Preserve staleness clock on merge — consolidated fact now carries the newest source's createdAt (not now) so aged information does not gain a fresh staleness-review window just by being consolidated; consolidatedAt is added as an explicit audit field. Three regression tests added (default=false, null-confidence consistency, createdAt policy); all guardrail tests now set consolidation_enabled=True explicitly so they test the guardrail, not the feature flag. 38 tests pass. * fix(memory): harden createdAt comparison and confidence handling 1. createdAt max via _parse_fact_datetime — replaces string max() which crashes on non-string createdAt (numeric unix timestamps) and sorts Z/+00:00 mixed formats incorrectly. Mirrors how staleness computes age. 2. Remove dead min(..., 1.0) — _coerce_source_confidence already clamps each source confidence to [0, 1], so max(source_confidences) ≤ 1.0 by contract; the outer min could never bind. 3. Clamp raw_llm_conf to [0, 1] before applying the source cap — out-of- range values like 1.5 are safe today (pinned by the cap) but defensively clamped first so the invariant holds even if the cap is ever loosened. 4. Doc: expand the apply-time guardrails comment to call out the protected- category exclusion via allowed_source_ids — this is the central safety property ("explicit user feedback is never silently merged away"). 5. Test: add test_confidence_fallback_to_max_source_when_llm_omits_field covering the else-branch (LLM omits confidence → uses max_source_conf). 6. Facts created long ago may become outdated without any future conversation
explicitly contradicting them ("Silent Staleness"). This adds a staleness
review mechanism that surfaces aged facts to the LLM during the normal
memory-update call so it can semantically judge whether each is still valid.
- New MemoryConfig fields: staleness_review_enabled, staleness_age_days,
staleness_min_candidates, staleness_max_removals_per_cycle,
staleness_protected_categories
- New STALENESS_REVIEW_PROMPT section injected into MEMORY_UPDATE_PROMPT
when enough stale candidates exist
- New staleFactsToRemove output field in the LLM response schema
- Safety cap limits max removals per cycle, keeping lowest-confidence
entries when the LLM returns more than the cap
- Correction facts (category=correction) are protected by default
- Observability via structured logging of each removal with reason
- 32 unit tests covering parsing, selection, triggers, formatting,
normalization, safety cap, and integration
* fix(memory): add deterministic guardrail for staleness removals
_apply_updates previously removed any fact id the LLM returned in
staleFactsToRemove without verifying it was in the actual staleness
candidate set. An LLM slip could silently delete protected-category
facts (e.g. correction) or fresh facts, defeating the stated guarantee.
Now intersect stale_ids_to_remove with _select_stale_candidates before
the safety cap, making the protection independent of both model behavior
and the staleness_review_enabled flag.
Add three regression tests:
- test_protected_category_fact_refused_at_apply
- test_non_aged_fact_refused_at_apply
- test_guardrail_runs_when_staleness_review_disabled
* docs(memory): sync AGENTS.md staleness config + simplify datetime parsing
Address reviewer feedback from PR #3860:
- Add staleness workflow step and 5 new config fields to backend/AGENTS.md
- Simplify _parse_fact_datetime: drop manual Z→+00:00 replace, Python 3.12+ fromisoformat handles Z natively

chore: remove unused private helper functions (dead code) (#3749)
These three module-private helpers have no callers anywhere in the repo (verified by a repo-wide identifier scan + direct greps; each appears only at its own definition): - agents/memory/updater.py: _create_empty_memory (a no-caller wrapper around storage.create_empty_memory, which is used directly elsewhere) - runtime/runs/worker.py: _extract_human_message (also drops the now-unused HumanMessage TYPE_CHECKING import and TYPE_CHECKING from typing) - sandbox/tools.py: _looks_like_unsafe_cwd_target (cwd safety is enforced via _is_allowed_local_bash_absolute_path) Underscore-prefixed = module-internal, so zero references is conclusive. Route handlers and @compiles-registered functions were excluded as false positives. Public-surface candidates are noted in the issue, not touched here. Fixes #3748. |

fix(memory): filter hide_from_ui HumanMessages from memory builder (#3697)
Middleware-injected hidden messages (TodoMiddleware.todo_reminder, ViewImageMiddleware, p0 DynamicContextMiddleware.__memory) were being fed to the memory-updating LLM as if they were real user input, polluting long-term memory with framework-internal text. The p0 __memory payload could also trigger a self-amplification loop. Fix: skip HumanMessages with additional_kwargs['hide_from_ui'] in filter_messages_for_memory, consistent with the frontend logic. Closes #3695 |
