feature(gateway): issue request trace ids unconditionally (#5119)
* refactor(gateway): issue request trace ids unconditionally

The request trace id was gated behind logging.enhance.enabled at every
entry point, so downstream code had to keep asking whether one existed:
a header-provenance flag in its own ContextVar, a precedence resolver,
and three-level carrier fallbacks at each consumer.

Bind one unconditionally instead. TraceMiddleware covers Gateway HTTP;
ensure_trace_context covers the entry points that never touch ASGI --
scheduled occurrences, MCP task notification runs, IM channel messages,
and the embedded client -- each scoped to one unit of work so a
long-lived worker task cannot leak one occurrence's id into the next.
The ContextVar becomes the only source; the response header, runtime
context, run metadata and log records are derived outputs.

Consumers now use ensure_trace_id() or resolve_trace_id(*carriers) and
drop their presence guards. Removed: resolve_deerflow_trace_id, the
header-provenance flag and its three helpers, set/reset_current_trace_id,
is_trace_correlation_enabled and its gateway alias.

BREAKING CHANGE: every Gateway HTTP response now carries X-Trace-Id and
it cannot be turned off; logging.enhance.enabled controls log output
only. Installations on the default enabled: false will start seeing the
header. No config keys were added or removed.

* fix(gateway): stop persisting a caller-supplied trace id on the run record

body.metadata forks two ways: through build_run_config into the live run
config, which the run worker restamps, and through create_or_reject into
the run record that the runs API echoes verbatim. Only the first was
covered, so a client sending metadata.deerflow_trace_id made the most
durable and most visible surface of a run disagree with the X-Trace-Id
and the log lines the same request produced -- a correlation id that
does not match the logs is worse than none.

Stamp the server-issued id once at the trust boundary so both forks
receive it, preserving the caller's own metadata keys. Close the same
gap on config.context, which reaches the runtime context by a separate
path: _build_runtime_context no longer merges server-owned keys from the
caller, and _install_runtime_context assigns rather than setdefaults.

A thread's metadata is no longer seeded with the run-scoped id of
whichever run created it -- one thread spans many runs and as many
trace ids.

Found by driving a real run through the Gateway and reading the run back
from the runs API; every unit test built its metadata by hand and so
could not see it.

* fix(gateway): expose X-Trace-Id to split-origin browser clients

X-Trace-Id is not on the CORS safelist, so a browser client served from
a separate origin could not read it -- and those are exactly the clients
that cannot read the Gateway's logs either, leaving them with nothing to
quote in a bug report. Same-origin nginx deployments were unaffected,
which is why this stayed hidden.

Add it to CORS_EXPOSED_HEADERS beside Content-Location, referencing
TRACE_ID_HEADER rather than repeating the literal.

* fix(gateway): keep X-Trace-Id on unhandled-exception 500s

Starlette's ServerErrorMiddleware sits outside every user middleware and
emits unhandled-exception 500s through the raw send, so those responses
never pass TraceMiddleware's header-writing wrapper. The 500 for a server
bug is exactly the response a user most needs to correlate with a log line,
and it was the one response that shipped without the id.

TraceMiddleware now tracks whether http.response.start has been sent. On an
exception with no response started it emits its own plain 500 carrying the
header, then re-raises: the outer ServerErrorMiddleware sees the response
already started and only re-raises too, so the server's exception logging is
untouched. An exception mid-stream keeps propagating unchanged — a second
response start cannot be sent, and the already-written header stands.

The trace id is printable ASCII by construction (normalize_trace_id /
generate_trace_id), which is what makes the raw latin-1 header encoding
safe.

* fix(gateway): strip the forged trace id from the persisted request echo

The run-record fix stopped a forged metadata.deerflow_trace_id on the
authoritative metadata surface, but the raw request echo still carried one:
create_or_reject persists body.config verbatim as runs.kwargs_json, which
the runs API serves back. A client posting config.context.deerflow_trace_id
therefore still got its forged value stored and echoed on one API surface
while the header, logs, run metadata, and checkpoint all carried the real
id — the id is ignored as input there, so echoing it back only manufactures
disagreement.

Two changes close it. redact_config_secrets — already the shared scrub for
that echo, applied at admission and again at serve time, so historical
records are covered too — now also drops deerflow_trace_id from
config.metadata and config.context. And build_run_config now merges run
metadata onto a copy of the caller's config["metadata"] instead of updating
it in place: the nested values of the request config are reference copies,
so the in-place merge was writing the server-stamped key through into
body.config, contaminating the "what the client sent" record before it was
persisted (and incidentally masking the forged-value echo on the metadata
container).

The regression test posts a forged id through body.metadata,
config.metadata, and config.context at once and reads the kwargs echo back
off the run record, failing if either leak returns.

* docs(harness): record the trace-echo scrub, 500 fallback, and accepted retry divergence

The trace section of the harness AGENTS.md now covers the two fixes that
close the derived-output rule (the kwargs-echo scrub in
redact_config_secrets plus build_run_config's copy merge, and
TraceMiddleware's own 500 for unhandled exceptions), and CHANGELOG gains
their Fixed entries.

It also writes down the one accepted divergence: a crash-recovered
scheduled launch reuses the durable run through its idempotency key, and
start_run returns early on idempotency_reused without restamping — so the
run record keeps the first attempt's deerflow_trace_id while the retry's
own log lines carry the freshly minted id of its ensure_trace_context
binding. The divergence is confined to the crash-recovery window and is
accepted rather than fixed: restamping on reuse would rewrite a persisted
record for a run that already exists, which is worse than two ids that each
correlate their own attempt's logs. Written down so the next reader of the
scheduler recovery path does not diagnose it as a bug.

* docs(config): align the logging.enhance schema note with the unconditional trace id

The config-module AGENTS.md still described logging.enhance as the gate for
the Gateway X-Trace-Id header and Langfuse deerflow_trace_id. That model is
gone: ids are issued unconditionally and this block decides log output only.
Left as-is, the stale wording invites an agent to "restore" a header gate it
believes was lost. Reworded to match the sibling AGENTS.md files and
config.example.yaml, with a pointer to the Request Trace Context section
that owns the full model.

* docs(changelog): link the trace entries to #5119

The five new entries pointed at the ([#XXXX]) placeholder with no reference
definition, rendering as literal text instead of a link — and RELEASING.md
step 2 relies on those references when the section becomes release notes.
All five now point at #5119, with the definition appended to the reference
block.

* refactor(harness): rename _stream_without_trace_context to _stream_turn

The name asserted the opposite of what the method now does. It was accurate
while logging.enhance.enabled could route stream() around the trace scope;
with the gate gone it is the only stream implementation left, and it binds
the id itself via ensure_trace_id(). Private, so the rename touches only the
definition and the one stream() call site.

* docs(harness): fit the trace-context guidance inside the AGENTS.md chain budget

The expanded Request Trace Context section pushed the effective AGENTS.md
chain for agents/middlewares to 99,815 bytes, past the 98,304 hard limit
scripts/check_agent_guidance.py enforces in CI (AG002). Compressed the
section from 7,359 to 4592 bytes with no facts removed: the entry-point
table, the derived-output rule and its enforcement points, the accepted
scheduled-retry divergence, the two resolution helpers, the stream()
binding rationale, the log-output-only gate, the CORS listing, the 500
fallback, and the test map all remain.

Sized against the merge, not just the branch: current main grew the same
chain by ~724 bytes, so the check was verified on the merged tree as well
(97,772 bytes; branch tree 97,048).

* fix(gateway): declare content-length on the fallback 500

The pre-response 500 declared content-type but no content-length, leaving
the framing to the ASGI server: chunked on HTTP/1.1, close-delimited on
HTTP/1.0 — the one wire difference from the ServerErrorMiddleware response
it replaces, which sends content-length: 21. The explicit header keeps the
fallback byte-identical to what clients saw before.

* docs(readme): drop the trace-correlation condition from the translations

The zh/ja/fr/ru Langfuse sections still said metadata.deerflow_trace_id
matches X-Trace-Id "when request trace correlation is enabled". The id now
always matches and that condition no longer exists, so each bullet states
the unconditional match and that logging.enhance.enabled only controls
whether the id is printed into logs — the one piece of the feature a user
can still configure.

* test(gateway): pin TraceMiddleware wiring through create_app()

Every X-Trace-Id test exercised a hand-built four-route app, so the real
stack's add_middleware(TraceMiddleware) line was pinned by nothing: deleting
it — or short-circuiting above it — passed CI while silently dropping both
the response header and the ambient id the run-record stamp and enhanced log
records derive from. One case now drives /health through create_app() and
asserts the inbound id round-trips; mutation-checked by removing the wiring
line, which fails exactly this test.

* docs(gateway): note the fallback 500 is CORS-opaque

The pre-response 500 is emitted outside CORSMiddleware — the exception has
already unwound past it — so it carries no Access-Control-Allow-Origin and
a split-origin browser client cannot read the id on this one response,
unchanged from the ServerErrorMiddleware 500 it replaces. Documented on the
class and in the CHANGELOG entry rather than fixed: replicating the origin
allowlist outside CORSMiddleware would let the two policies drift.

* fix(harness): keep abandoned-stream cleanup inside the trace binding

stream() binds the turn's id around each next(inner) and resets it before
yielding, but the finally's inner.close() ran after that binding was gone.
Abandoning the stream therefore drove the inner LangGraph generator's
GeneratorExit/finally path with no trace id — or an unrelated ambient one
from whichever context ran the close — so cancellation and finalization
logs and callbacks did not correlate with the turn they belong to.

inner.close() is now wrapped in a local bind/reset of the same turn id. The
token is set and reset in the same frame, never across a yield, so the
per-step cross-context safety is preserved even when GC closes the
generator from another Context — pinned by the existing copy_context close
test, which now exercises this path. The regression test records the id
from the inner generator's finally and fails without the binding.

* test(harness): teach the worker-trace fake about RunManager.cleanup

Upstream #5112 (bound gateway memory after terminal runs) added a
run_manager.cleanup(run_id) call to run_agent's finalization, so the
merge-commit CI run failed all five worker-trace-binding tests with
AttributeError on this PR's _FakeRunManager. The fake gains the same no-op
shape as its other methods.

* docs(gateway): bring the gateway AGENTS.md back under its soft budget

Upstream #5092 grew backend/app/gateway/AGENTS.md to 40,966 bytes, 6 over
the 40,960 soft budget that
test_agent_guidance_check.py::test_repository_guidance_stays_below_soft_budgets_and_avoids_doc_indexes
enforces — its Unit Tests run on main was cancelled by push concurrency, so
main is currently red on that test and every PR merge-run inherits the
failure. Two whitespace/wording trims in the row #5092 touched (a doubled
space, and "its configured `context_window`" → "its `context_window`")
bring the file to 40,953 with no content change.

* feat(harness): add deterministic tool receipts with model-visible ledger

Stamp an immutable per-call fact record (tool name, status, args/output
hashes, byte count, timestamp) onto every tool result via a new
ToolReceiptMiddleware, and inject the derived receipt ledger (r1..rN)
into the model context so subagent reports can cite executed actions.

- tool_receipt.py: receipt core (make/extract/render), newest-first
  budget eviction, ids derived from the append-only message stream
- ToolReceiptMiddleware: stamps ToolMessages directly or inside
  Command-wrapped results; hidden ledger injection mirrors
  DurableContextMiddleware; sits between ToolProgress and
  ToolErrorHandling with a build-time ordering guard
- config: new verification section (receipts on, judge off), config
  version 32 -> 33 with example/helm/docs updates

* feat(harness): split receipt rendering from stamping; address PR review

Review fixes (PR #4659):
- output_sha256 now uses sort_keys=True for structured content, matching
  the order-invariant args fingerprint
- stamping failures log at warning (silent ledger gaps would corrupt

main reached 34 via #4780 without the verification section; publishing
the new schema at the same number would silently skip the outdated-config

upstream/main adf6c422 uses errno.ENOTDIR in the drift guard but removed
the import, so the PR merge ref fails lint-backend (F821).

* fix(harness): harden tool receipts against forgery and turn-scope delegation_only

Address willem-bd's pre-merge review on #4659:

1. Untrusted receipt metadata: the gateway now strips the server-owned
   deerflow_tool_receipt key from external input messages; stamping always
   overwrites any tool-supplied value instead of preserving it; and
   extract_tool_receipts validates persisted receipt shapes (required typed
   fields, unknown keys ignored) so malformed entries are skipped instead of
   crashing render or passing as runtime-stamped evidence.

2. delegation_only no longer sticks on: _should_render now scopes the
   subagent_status scan to the current turn (messages after the latest
   genuine user message), so an old completed delegation stops rendering the
   ledger on later ordinary turns. The genuine-user predicate moves to
   message_utils.is_genuine_user_message, shared with input sanitization.

* fix(harness): stamp receipts outside short-circuiting tool middlewares

Address willem-bd's review on #4659: ToolReceiptMiddleware was registered
inside Guardrail/SandboxAudit/ReadBeforeWrite/ToolProgress, each of which
can return a ToolMessage without invoking its handler — blocked calls
(e.g. a read-before-write-denied write_file) never got a receipt, silently
gapping the ledger on a default-enabled path. SandboxAudit additionally
rebuilds medium-risk results, dropping an inner stamp.

ToolReceiptMiddleware is now the outermost wrap_tool_call layer in the
runtime tail. Normal results still carry deerflow_tool_meta (stamped by
ToolErrorHandling on the inner return path); short-circuit messages
self-stamp meta or fall back to message.status. The new invariant is
declared as ordering constraints in deerflow.extensions.ordering, with
composed-chain regression tests for a blocked write and a warn-rebuilt
bash result.

poll_attempt_count grows on every claim (successful polls included), so it
cannot drive a failure backoff without misjudging normal long tasks. Add
consecutive_poll_error_count: incremented when a claim is released after a
poll error, reset to zero by any applied snapshot. The backoff/terminal
policy that consumes it lands with the first concrete driver.

* fix(extensions): stop config resolution from masking extension loading

`create_app()` resolved the configured plugin list inside the fail-open
guard around `load_extensions()`. CI has no `config.yaml` (gitignored and
never generated by the workflow), so `get_app_config()` raised
`FileNotFoundError` there and was swallowed as an extension failure --
`load_extensions()` never ran at all, and the four `create_app()` tests in
`test_extension_app_loading.py` passed locally but failed on every runner.

Resolve the plugin list before the guard. Only an absent `config.yaml` is
tolerated, mirroring `_resolve_trace_enabled_for_app_construction()`:
`create_app()` runs at import time, and lifespan still performs strict
config loading before serving. A `config.yaml` that exists but fails to
parse or validate now propagates instead of being reported as an extension
failure -- reporting it as the latter silently dropped a `required: true`
extension rather than failing the boot.

Make the tests config-independent with an autouse `stub_app_config`
fixture, following the existing pattern in `test_gateway_lifespan_shutdown.py`,
and cover both new branches of the config-resolution boundary.

* fix(extensions): bind the run's extension snapshot through subagent delegation

The lead-agent path resolves one immutable loaded-extension snapshot per run
and binds it through task-store allocation and graph construction, but the
subagent path re-read the process-wide singleton at execution time. In
production both are the same object, yet a `set_loaded_extensions()` between
the lead run's start and a subagent's execution (test teardown, a future
hot-reload path) would let one run mix two extension generations — exactly what
the documented invariant exists to prevent.

The graph-build binding is a ContextVar scoped to synchronous construction, so
it has already exited by the time a tool delegates; the snapshot has to travel
through runtime context instead. The run worker publishes it under the
host-internal `EXTENSION_SNAPSHOT_CONTEXT_KEY` (written after the caller merge,
popped when the run has none, so a caller-supplied value is never
authoritative), `task_tool` reads it back through the type-checking
`resolve_run_extensions()`, and `SubagentExecutor` binds it at construction.

Callers outside the Gateway run path — embedded `DeerFlowClient`, standalone
LangGraph Server — install no snapshot and keep the existing
`get_loaded_extensions()` fallback.

* refactor(extensions): defer the ordering table by call, not by a lying tuple

`CORE_ORDERING_CONSTRAINTS` was a `tuple` subclass that overrode only
`__iter__` and resolved into a class-level `_resolved` side channel. A tuple
cannot populate its own storage after construction, so the instance stayed the
empty tuple it was built as: `len()` was 0, `bool()` was False, `in` was always
False, indexing raised, slicing and `reversed()` came back empty, and it
compared unequal to the plain tuples tests substitute for it — all while
iteration yielded the real constraints. Only `assert_ordering` consumed it, and
only by iterating, so the split went unnoticed.

The sibling `_AnchorTable(dict)` uses the same idea soundly because dict is
mutable: `self.update()` fills the real storage, making every inherited
operation correct. That trick does not survive the port to an immutable type.

Replace it with `core_ordering_constraints()`, matching how `stack.py` defers
the same kind of table via `_anchors()`. The deferral is kept — it is about
dependency direction, not just cycles: `extensions/` is the layer the
middleware layer calls into, so a module-scope `agents.middlewares` import here
points the dependency backwards and closes a cycle as soon as any middleware
imports something under `extensions/` at module level. Resolution stays at
`assert_ordering` time, which already runs inside the middleware builder.

Tests pin both halves: the returned value is a plain tuple whose len/bool/
membership/indexing/reversal/equality agree with iteration, and a subprocess
probe asserts importing `extensions.ordering` does not load the middleware
layer while calling the function does.

* fix(persistence): address pg-schema review feedback

- Restrict postgres_schema regex to lowercase-only so the quoted CREATE
  SCHEMA matches the unquoted search_path (PG case-folds it), fixing the
  mixed-case bug where tables silently fell back to public.
- Replace shlex.join/split with libpq-correct backslash escaping for the
  options parameter so values containing spaces survive intact.
- Add normalize_libpq_dsn() and route the async checkpointer pool through
  dsn_with_search_path() so a +asyncpg suffix is stripped and existing DSN
  options (e.g. statement_timeout) are merged instead of overridden.
- Extract shared ensure_postgres_schema()/ensure_postgres_schema_async()
  helpers (mapping missing psycopg to the install hint) used by all four
  provider sites.
- Tests: reject mixed-case schemas, preserve space-containing libpq option,
  cover normalize_libpq_dsn, and assert pool search_path via DSN.

* fix(persistence): align pg-schema test with merged store API

The main merge moved the sync Store factory to the single-path
_resolve_store_config/_sync_store_cm design, dropping the PR's
_sync_store_from_database helper. The integration test still imported
the removed symbol, breaking test collection (backend-unit-tests).
Resolve the store config from a DatabaseConfig and drive it through
_sync_store_cm instead.

* fix(persistence): address pg-schema review feedback

- reject trailing/leading whitespace in postgres_schema via re.fullmatch
  (a $-anchored re.match let "deerflow\n" through, silently landing tables
  in public)
- re-escape all whitespace (TAB/CR/LF) when re-joining libpq options so a
  caller's pre-existing options value round-trips losslessly
- re-validate the identifier inside create_schema_sql as defense-in-depth
  at the SQL-emitting boundary
- accept the postgres:// short scheme in the alembic search_path injection
- close the sync psycopg connection explicitly (psycopg3 __exit__ does not
  close()), mirroring the async path
- drop the partial checkpointer/store reset on a database config change;
  database is restart-required and the ORM engine is not rebuilt, so a
  partial reset would half-migrate the deployment

* docs(config): complete the postgres_schema migration checklist

Address PR review (P1): the documented `public`->schema migration only
moved runs, run_events, threads_meta, feedback, and users. That strands
every other DeerFlow-owned table -- the four channel_* tables, both
scheduled_* tables, agents, and (critically) alembic_version -- in
`public`. On restart bootstrap treats the partially-populated target
schema as unversioned, re-baselines it, and replays migrations while the
real rows stay invisible in `public`.

List the full owned set explicitly, call out alembic_version as required,
and keep the "discover the rest" query for version-drift safety.

* refactor(checkpointer): drop test-only _sync_checkpointer_from_database

Address PR review: the helper was only reached by the env-gated
integration test and re-implemented the DatabaseConfig->CheckpointerConfig
backend resolution that _resolve_checkpointer_config already owns, so a
future backend added there would silently miss this path. Mirror the store
side of the same test, which reuses the production path directly:
_resolve_checkpointer_config(...) + _sync_checkpointer_cm(...).

Add an agent_storage.backend switch (default file, behaviour-unchanged) with a
db backend that stores each custom agent as a row in the shared SQL persistence
layer, so a multi-instance deployment sees the same agents on every node
(#4331, #4357). Introduces an AgentStore interface routing all read/write
surfaces, an agents table + migration 0006, startup validation, and a file->db
importer. Follows the thread_meta store / run_events backend-switch /
0003_scheduled_tasks migration patterns; no new dependency.

* fix(agents): make db storage path production-ready (review round 1)

Addresses review feedback on the db/sync agent-storage path:

- sql.py: mirror the async engine's per-connection SQLite PRAGMAs on the sync
  engine (busy_timeout=30000, synchronous=NORMAL, foreign_keys=ON, WAL) so both
  engines behave identically against the shared DB; guard the engine cache with
  a lock (double-checked) so concurrent first-touch cannot build duplicate
  engines or register the connect listener twice.
- routers/agents.py + routers/assistants_compat.py: offload the sync-store reads
  that ran on the event loop (list/get/check, update's pre-read + legacy guard +
  refresh, and assistants_compat's four list routes) via asyncio.to_thread — on
  db+postgres each was a network round trip stalling the loop. Writes were
  already offloaded.
- file.py: translate the create() mkdir(exist_ok=False) race FileExistsError
  into AgentExistsError (router 409, matching SqlAgentStore's IntegrityError
  path); correct the _write docstring — per-file atomic replace, two commits
  sequential not transactional.

Tests: sync-engine PRAGMA + engine-cache reuse assertions; file create-race ->
AgentExistsError; strict Blockbuster anchor over the read endpoints so a
regression back onto the loop fails CI.

* fix(agents): address round-2 review on the db store path

- update_agent tool: align the docstring/inline comment with FileAgentStore._write.
  Cross-field write atomicity is db-only; the file backend commits config then
  soul via two sequential os.replace (a crash between them can leave a fresh
  config.yaml beside a stale SOUL.md). The dropped partial-write *reporting* is
  an intentional tradeoff — the stage-then-replace safety is preserved
  (test_update_agent_soul_failure_does_not_replace_config still holds).
- SqlAgentStore.update(): true upsert. Catch IntegrityError on the
  insert-on-missing branch, re-fetch and apply, so two concurrent first-time
  writes (e.g. two setup_agent handshakes) converge instead of surfacing a raw
  UNIQUE(user_id, name) violation as a 500. Symmetric with create().
- get_agent_store(): document the graph-subprocess config-resolution invariant
  (the except->file fallback is a genuine no-config path, not a mask for a
  misconfigured graph process) and pin it with two tests driving the real
  get_app_config() file resolution: db resolves from an on-disk config.yaml,
  file fallback when config is unresolvable.

* test(agents): cover SqlAgentStore.update() write-race upsert recovery

Mandatory-TDD test for the round-2 fix in 0680340a: two concurrent first-time
update()s where the loser's insert hits UNIQUE(user_id, name). Deterministically
forces the IntegrityError recovery path by making the first _row probe miss the
committed winner, and asserts last-writer-wins instead of a surfaced 500.

P1 #1: the asyncio.Semaphore limiter was loop-bound, so it recreated per
event loop and the cap was NOT process-wide: lead-agent calls (main loop)
and subagent calls (the isolated persistent loop in subagents/executor.py)
each got their own semaphore, and the sync graph path (wrap_model_call)
bypassed the cap entirely. Recreating on loop/limit change also abandoned
permits held by the prior instance.

Replace it with a _ProcessWideLimiter built on threading primitives (not
loop-bound): one limiter shared across every event loop and both sync/async
wrappers. The cap is mutable via set_limit (never recreates, so in-flight
permits are never abandoned); permits release in finally and async waiters
unregister on cancellation, so cancellation never leaks capacity. Wire it
into wrap_model_call (sync) too - previously a direct handler() call.

P1 #2: the first (and only) burst-rate retry was deterministic at 5000ms.
prev_delay_ms was seeded from the 1000ms normal base, so for burst the
window collapsed to randint(5000, max(5000, 1000*3)) = randint(5000, 5000) -
a fleet that failed together realigned on the same 5s tick. Seed the first
retry from the reason-specific base (prev_delay_ms=None on loop init) so
the burst window is [burst_base, cap] = [5000, 8000], non-degenerate.
Retry-After is still honored verbatim.

Tests: rename semaphore tests -> limiter; add an autouse fixture resetting
the process singleton; add regressions the reviewer asked for - cross-loop
(lead + isolated-loop subagent), two concurrent sync calls, limit-change
while a permit is held (same instance, permit preserved), cancellation
no-leak, and burst first-retry non-degeneracy with default config (real
and seeded RNG) plus a concurrent de-synchronization case. Verified the
burst guard goes red on the old logic ({5000}) and green on the new.

frozen cap. Removes _owner_seq / set_limit_if_newer / _grant_to_queued_locked
/ _next_instance_seq; file 946 -> 926 lines.

P2 - burst-rate calls are capped at 2 attempts but the retry log line, the
llm_retry stream event max_attempts, and the user-facing message still used
self.retry_max_attempts (3), so the frontend showed 1/3 then stopped after
attempt 2. Thread the effective max_attempts (_max_attempts_for) through the
logger, _emit_retry_event, and _build_retry_message.

Also: document max_concurrent_calls as startup-only in the config field
description and config.example.yaml (prose only - the startup-only: prefix is
top-level AppConfig-field granularity and would mislabel the otherwise
hot-reloadable llm_call section / break the reload_boundary drift test).
Rewrite the cap-mutation tests for startup-only semantics; add P2 retry-budget
event tests (sync+async, teeth-verified red on the bug); fix bot nits (empty
except blocks -> gather(return_exceptions=True); bare await statements ->
assigned+asserted).

Extracts the (mtime, size, sha256) content-signature helper that was
duplicated between config/app_config.py and mcp/cache.py into a new
config/file_signature.py, and fixes ExtensionsConfig.resolve_config_path()
to return None instead of raising FileNotFoundError when an explicit
config_path argument or DEER_FLOW_EXTENSIONS_CONFIG_PATH points at a file
that has since been deleted -- the exact resolution mode Docker dev/prod
uses per AGENTS.md, so the MCP tools-cache staleness check could raise
instead of degrading to "not stale". Both were flagged by willem-bd in
review on #4124 and explicitly deferred there ("flagging for visibility",
"leaving the extraction as the follow-up you suggested").

* fix(config): keep explicit extensions-config paths fail-loud

resolve_config_path() previously turned every missing-file case into a
clean None, including an explicit config_path argument or
DEER_FLOW_EXTENSIONS_CONFIG_PATH (the exact mode Docker dev/prod uses).
That silently downgrades a bad Docker mount, typo, or deleted
production config to "no extensions" instead of surfacing the
misconfiguration, per fancyboi999's review [P1] and willem-bd's
follow-up notes on this PR.

Restores FileNotFoundError for the two explicit modes (config_path
argument, DEER_FLOW_EXTENSIONS_CONFIG_PATH); only the fallback search
mode (no explicit path/env var, nothing found in the usual locations)
still returns None, since that is the legitimate "extensions were
never configured" case.

The one caller that needs the old fail-soft behavior -- the MCP
tools-cache staleness check, which re-resolves the path on every
get_cached_mcp_tools() call -- gets a narrow, local catch instead
(deerflow.mcp.cache._resolve_config_path) so a config file going
missing mid-run still degrades the cache to "not stale" rather than
crashing a hot per-request path. Also hoists the double os.getenv()
read in the env-var branch into a local, per willem-bd's nit.

Adds resolver-level tests for both explicit-path and env-var raises,
a dedicated search-mode None regression test, and updates the
existing MCP-cache docstrings to describe the corrected split.

- GuardrailAuthorizationAdapter: bridges AuthorizationProvider → GuardrailProvider
  so existing GuardrailMiddleware can enforce authz decisions without a new middleware

New authorization config section (default enabled: false):
- AuthorizationConfig wired into AppConfig alongside guardrails
- Singleton load/reset mirrors GuardrailsConfig pattern
- config.example.yaml documents the RBAC provider schema

29 tests covering protocol conformance, dataclass construction, adapter
request/decision mapping, and config singleton behavior.

Per RFC #4063 Phase 0 (foundations). Layer 1/2 wiring and Principal builder

fix(runs): close multi-worker ownership gaps in run atomicity (#3948) (#4003)
* feat(runs): cross-process run ownership with lease + reconciliation (#3948)

Implements work items 2 and 3 of the multi-worker P0 plan
(docs/multi_worker.md). Work item 1 (Postgres startup gate, #3960)
already landed; this PR makes run creation race-safe across worker
processes and lets Postgres deployments recover orphaned inflight runs
from crashed workers without mis-marking live runs as orphans.

Work item 2 — cross-process atomic create_or_reject

- Alembic revision 0004_run_ownership adds runs.owner_worker_id,
  runs.lease_expires_at, idx_runs_lease, and a partial unique index
  uq_runs_thread_active (one pending/running run per thread). The
  index is declared on RunRow.__table_args__ with sqlite_where +
  postgresql_where (mirroring uq_channel_connection_active_identity)
  so the empty-DB bootstrap path — which runs Base.metadata.create_all
  + alembic stamp head without executing any revision's upgrade() —
  also lands it on fresh deployments. Migration 0004 additionally
  creates it idempotently for legacy/versioned upgrades.
- RunRepository.create_run_atomic is the new atomic primitive:
  - reject: INSERT directly; the partial unique index catches
    duplicate active runs; the manager surfaces the result as
    ConflictError.
  - interrupt/rollback: SELECT FOR UPDATE the conflicting rows,
    skip rows whose lease is still valid AND owned by another live
    worker (raise ConflictError — the INSERT would have failed on
    the index anyway, and a retry loop cannot make progress),
    cancel the rest in the same transaction, then INSERT the new
    row. Rows owned by this worker are interruptible regardless of
    lease state.
- RunManager.create_or_reject dispatches to the store under the
  existing local lock; same-worker in-memory cancellation runs after
  the store commit succeeds. MemoryRunStore mirrors the same
  semantics for tests and database.backend=memory.

Work item 3 — lease heartbeat + Postgres reconciliation

- RunOwnershipConfig (lease_seconds=30, grace_seconds=10,
  heartbeat_enabled=false by default), registered as startup-only in
  reload_boundary.STARTUP_ONLY_FIELDS because the heartbeat background
  task is created once in langgraph_runtime() and is not rebuilt on
  config.yaml edits.
- When heartbeat_enabled, each worker renewes leases on its own
  active runs with interval = lease_seconds / 3. The loop is bounded
  and stop-event-cancellable so shutdown is prompt.
- reconcile_orphaned_inflight_runs now runs on every backend — the
  sqlite-only gate in app/gateway/deps.py is dropped in the same
  commit so there is no window where Postgres would mis-mark live
  Worker A runs as orphans. Reconciliation errors only runs whose
  lease is NULL (legacy pre-ownership rows) or older than
  grace_seconds. In single-worker mode (heartbeat off, NULL leases)
  all inflight rows reclaim immediately, preserving the pre-ownership
  recovery latency.
- Heartbeat starts AFTER startup reconciliation and stops BEFORE the
  in-flight run drain on shutdown so the two cannot race.

GATEWAY_WORKERS=1 with heartbeat_enabled=false keeps current behavior.

Verified: 170 related tests + full backend suite (minus Docker-gated
live tests) green; ruff check + ruff format clean.

* fix(runs): tighten unique-violation handling and document clock-sync budget

Three follow-up fixes to the cross-process run ownership work in #3948,
surfacing during review.

1. _is_unique_violation: detect by driver-native signal, not message text

   The previous substring heuristic ("unique" + "violat", or "duplicate")
   missed SQLite's actual phrasing "UNIQUE constraint failed: <table>.<index>"
   — SQLite says "failed", not "violates", and never "duplicate". On SQLite
   the detector returned False, the reject path re-raised the raw
   IntegrityError, and clients saw HTTP 500 instead of ConflictError 409.
   The conversion is the load-bearing piece of the "store is source of
   truth" design but was untested — every atomic test used MemoryRunStore,
   which raises ConflictError directly and never reached this branch.

   Now prefers driver-native signals: psycopg pgcode/sqlcode "23505" and
   sqlite3 sqlite_errorcode SQLITE_CONSTRAINT_UNIQUE (reachable through
   SQLAlchemy IntegrityError.orig). Message matching stays as a fallback
   with SQLite's exact "unique constraint failed" phrase added.

2. interrupt/rollback: convert exhausted-retry IntegrityError to ConflictError

   The reject branch converts unique violations to ConflictError. The
   interrupt/rollback retry loop did not — on the 3rd attempt it re-raised
   the raw IntegrityError, leaking HTTP 500 for the same race condition
   that reject surfaces as 409. Symmetric conversion added after the loop;
   callers now see a consistent ConflictError regardless of strategy.

3. Document clock-sync requirement for multi-worker lease reconciliation

   reconcile_orphaned_inflight_runs compares another worker's UTC
   lease_expires_at against this worker's datetime.now(UTC). The only skew
   budget is grace_seconds (default 10s) — worst case, with the owning
   worker's heartbeat just about to fire, a peer whose clock is more than
   ~grace_seconds ahead can mis-reclaim a still-live run as an orphan.

   Documented in RunOwnershipConfig's docstring (with the math) and in
   config.example.yaml (with operational guidance), so operators in
   NTP-poor environments know to raise grace_seconds. Default unchanged:
   10s is reasonable for NTP-synced K8s/cloud, and bumping it would slow
   recovery of genuinely dead workers (lease_seconds + grace_seconds from
   last heartbeat to reclaim).

Tests:
- test_create_run_atomic_reject_propagates_conflict_on_unique_violation:
  end-to-end against a real SQLite-backed RunRepository, pre-inserts an
  active run, asserts reject-strategy create surfaces as ConflictError
  rather than raw IntegrityError.
- test_is_unique_violation_detects_real_sqlite_integrity_error: unit test
  for the detector against a real SQLite-raised IntegrityError; asserts
  driver-level sqlite_errorcode is SQLITE_CONSTRAINT_UNIQUE.
- test_interrupt_exhausted_retries_surface_as_conflict_error: pins the
  symmetric 409 behavior after the retry loop exhausts.

Verified: ruff check + ruff format clean; multi-worker + run_repository
+ owner_isolation + reload_boundary suites green.

   agent task; under event-loop load that window can approach
   ``lease_seconds``, after which peer reconciliation marks the run
   ``error`` (visible) or a peer's ``create_or_reject("interrupt")``
   silently kills the queued run. Filter now:
   ``task is None or not task.done()``.

3. Document the unsynchronised ``record.lease_expires_at = new_expiry``
   write. ``lease_expires_at`` is the only field on an existing record
   this path mutates; ``set_status`` / ``_persist_status`` touch other
   fields, so there is no concurrent writer to race against. Re-acquiring
   ``self._lock`` would serialise unrelated run mutations for no gain.

4. Gate ``_is_unique_violation`` message fallbacks on
   ``isinstance(current, (SAIntegrityError, sqlite3.IntegrityError))``.
   The driver-code path (pgcode/sqlite_errorcode) remains load-bearing;
   substring fallbacks are now belt-and-suspenders only for cases where
   the driver attribute isn't reachable through the cause chain. Without
   the gate, any application exception whose ``str()`` happens to contain
   "duplicate key" / "unique" + "violat" (CHECK constraint, validation
   error) would silently surface as HTTP 409 instead of 500.

5. Route ``update_lease`` through ``_call_store_with_retry`` for
   consistency with every other store call, and wrap
   ``await self._renew_leases()`` in ``_heartbeat_loop`` with
   ``except Exception: logger.warning(...)``. Previously a transient
   error from the snapshot path or an unexpected exception would kill
   the heartbeat task silently — after which no lease is ever renewed
   again and every active run eventually looks orphaned.
   ``except Exception`` lets ``CancelledError`` (BaseException since
   3.8) propagate so shutdown cancellation still works.

Regression tests:
- ``test_heartbeat_renews_pending_run_before_task_is_spawned``
- ``test_is_unique_violation_does_not_misclassify_application_exception``

  ConflictError raised on a later candidate does not leave earlier candidates
  half-interrupted. Mirrors the SQL store's transactional rollback semantics;
  the entire test_multi_worker_run_ownership.py suite runs against memory so
  this divergence was giving false confidence.

- RunRepository.create_run_atomic interrupt path: coerce tz-naive
  ``row.lease_expires_at`` to UTC before comparing against the aware
  ``cutoff``. SQLite drops tzinfo on read despite ``DateTime(timezone=True)``
  (this file's own comment acknowledges it), so the Python-side comparison
  raised ``TypeError: can't compare offset-naive and offset-aware datetimes``
  whenever heartbeat was enabled on SQLite and a lease was non-NULL. Defaults
  (heartbeat off -> leases always NULL) masked it, but there was no guard
  against the combination. Follows the existing "naive is UTC" convention
  from ``coerce_iso``.

Each fix ships with a regression test pinning the behavior.

Three fixes from code review:

1. Extend the startup gate (GATEWAY_WORKERS>1) to also require

   or scale-up.

   and handle tz-naive lease values uniformly with the SQL store.

3. Store layer (sql.py, memory.py) now lazy-imports ConflictError

- Add a cancel affordance to the in-flight polish status pill that calls

  as the only escape.
- Reset promptHistoryIndexRef/promptHistoryDraftRef when a rewrite is applied
  (and on undo), so a stale history-browse index can no longer let the next
  ArrowDown silently overwrite the polished draft.
- Disable polishing while an open human-input card is present, matching the
  frontend/AGENTS.md rule that composer entry points defer to the card so
  card-reply metadata is preserved.
- canPolishInput now reuses parseGoalCommand/parseCompactCommand instead of a
  third hardcoded reserved-command regex, and drops the phantom /help entry
  (no /help parser exists in the composer), so future builtins only need to be
  taught to the existing parsers.

Backend
- Extract the non-graph one-shot LLM path (build model + inject Langfuse
  metadata + system/user invoke + text extract) into
  deerflow.utils.oneshot_llm.run_oneshot_llm, shared by the input-polish and
  suggestions routers so tracing-metadata and invocation shape cannot drift
  between the two copies.
- strip_think_blocks gains truncate_unclosed (default True, preserving the
  suggestions/goal JSON-prep behavior); input polish passes False so a draft
  that legitimately contains a literal <think> substring is no longer
  truncated into a partial rewrite or a spurious 503.
- Validate the empty-check and max_chars boundary against the same stripped
  view of the draft that is sent to the model, so the user-facing length
  boundary and the model input can no longer disagree.

Tests / docs
- Backend: literal-<think> preservation, whitespace-only rejection, and
  normalized-length/model-input agreement cases; suggestions tests repoint the
  create_chat_model patch to the shared helper module.
- Frontend: helper unit tests updated for the /help/reserved-command change; a
  new Playwright case covers cancelling an in-flight polish request.
- backend/AGENTS.md documents the shared one-shot helper and the polish
  normalization/think-tag behavior.

- Dead code: collapse _decode_text_for_analysis to a single decode path and

fix(gateway): clamp client-supplied recursion_limit to prevent runaway runs (#3903)
build_run_config() copied every top-level request key (except
configurable/context) verbatim into the LangGraph RunnableConfig, including
recursion_limit. The server default of 100 was fully overridable by the caller
with no upper bound, so a request like {"config": {"recursion_limit": 100000000}}
could make a single run execute effectively unbounded LangGraph super-steps
(each >= 1 LLM call), enabling runaway API cost / DoS.

Validate the client value server-side and clamp it into a safe range:
- valid positive ints are capped at a configurable ceiling
  (AppConfig.max_recursion_limit, default 1000 to match the existing
  frontend/public-skill default so legitimate deep runs are unaffected)
- invalid/non-positive/bool/None values fall back to the 100 server default
- applied on both the configurable and the LangGraph >= 0.6.0 context paths
- WARNING logged on clamp for observability

Add unit tests (including a configurable-ceiling case), expose
max_recursion_limit in config.example.yaml (config_version 16), and document
the ceiling/fallback in backend/docs/API.md.

perf(config): index AppConfig.get_*_config by name (O(n) -> O(1)) (#3688)
get_model_config / get_tool_config / get_tool_group_config did a next(...)
linear scan of self.models / self.tools / self.tool_groups on every call.
These sit on hot paths: get_tool_config runs 2-3x per community-tool
invocation (web_search etc.) and get_model_config several times per agent
build, across 40+ call sites.

Build name -> config dicts once in a mode="after" validator (stored as
PrivateAttr), so each getter is an O(1) dict lookup. A config reload
constructs a fresh AppConfig, which rebuilds the indexes; setdefault keeps
first-match-wins on duplicate names, matching the prior next(...) semantics.

#3434 made commented-out list sections (models/tools/tool_groups) parse as []
instead of crashing, but the same scenario still crashes for object sections:
commenting out every key under e.g. memory: / summarization: / guardrails:
makes PyYAML parse the value as None, and AppConfig then raises "Input should
be a valid dictionary" for that section — breaking the documented
`cp config.example.yaml config.yaml` first-run flow.

Generalize the handling: a model_validator(mode="before") drops None-valued
sections so each field falls back to its default (list sections -> [], object
sections -> their default config). This subsumes the previous list-only
field_validator and the database special-case. Required sections without a
default (sandbox) still error when null.

Adds test_app_config_coerces_commented_out_object_sections; the existing
list-section regression test still passes.

* docs+test: address review on null-section coercion

- Correct the _drop_null_config_sections docstring: it does not subsume the
  database special-case; _apply_database_defaults still owns `database` and
  applies concrete defaults beyond null-coercion.

fix(config): coerce null config.yaml list sections to empty list (#3434)
Copying config.example.yaml to config.yaml and starting DeerFlow crashed with `pydantic ValidationError: models — Input should be a valid list [input_value=None]`, because the example ships every entry under `models:` commented out, so PyYAML parses the key as null. Reported in #1444.

Add a field_validator(mode="before") on AppConfig that coerces null models/tools/tool_groups to [] (matching their default_factory=list), and emit an actionable warning from from_file when no models are configured (pointing to config.example.yaml / make setup). Adds regression tests.

* fix(config): make the reload boundary discoverable from code, not just docs

Closes #3144.

The hot-reload contract — per-run fields are resolved through
`get_app_config()` on every request, infrastructure fields snapshot at
gateway startup — landed in `backend/CLAUDE.md` as part of #3131. A
maintainer reading `get_config()` or an `AppConfig` field still had to
context-switch to that document to know which fields require a process
restart, and there was no enforcement that the prose list stayed in
sync with the code.

This commit moves the boundary to a machine-readable single source of
truth and surfaces it where the code lives:

- New `deerflow.config.reload_boundary` module owns the registry of
  restart-required fields (`STARTUP_ONLY_FIELDS`) and a tiny helper
  API (`is_startup_only_field`, `iter_startup_only_field_paths`,
  `format_field_description`). The standardised `"startup-only:"`
  prefix is exported as `STARTUP_ONLY_PREFIX` so future scanners /
  lint hooks / doc generators can pivot off it without re-parsing
  prose.
- `AppConfig`'s `database`, `checkpointer`, `run_events`,
  `stream_bridge`, `sandbox`, and `log_level` fields now build their
  `Field(description=...)` from `format_field_description(...)`. The
  same text shows up in IDE hover (Pydantic v2 exposes `description`
  via `model_fields[...]`).
- `channels` is restart-required too but lives outside the AppConfig
  Pydantic schema (the config section is consumed directly by
  `start_channel_service`). The registry owns it so the boundary is
  not split between two places.
- `get_config()` docstring points to the registry instead of leaving
  the reader to find `CLAUDE.md`. The `CLAUDE.md` table collapses to
  a one-liner pointing back at `reload_boundary.py` so the boundary
  has one canonical location, not two.

Drift coverage in `tests/test_reload_boundary.py`:

- Every registered field has a non-trivial reason.
- Iterator / membership helpers stay in sync with the dict.
- Every registry entry that maps to an `AppConfig` field also carries
  the `"startup-only:"` prefix in the schema (catches "forgot to
  update the schema").
- Reverse drift: any AppConfig field whose description starts with
  the prefix must be registered (catches "marked restart-required in
  the schema but forgot the registry").
- The runtime introspection that IDE hover depends on
  (`AppConfig.model_fields["database"].description`) is pinned, so a
  future Pydantic upgrade or schema swap that breaks the hover surface
  shows up as a test failure rather than a silent regression.

Refs: bytedance/deer-flow#3138 (split summary), #3107 (origin), #3131
(prior boundary fix in prose form).

* fix(config): preserve field doc and correct log_level reload reason

Two follow-ups on the PR #3153 review:

1. The `log_level` STARTUP_ONLY_FIELDS reason previously claimed
   `apply_logging_level()` mutates the root logger level. It does not:
   only the `deerflow` / `app` logger levels are set, and root handler
   thresholds are conditionally lowered so messages from those loggers
   can propagate. Reword to match the actual behavior so operators
   reading IDE hover get accurate restart guidance.

2. `format_field_description(field_path)` was the sole `Field(description=)`
   for every restart-required field, which silently overwrote the
   original human-facing documentation — most visibly the `log_level`
   field that used to list debug/info/warning/error and clarify that
   third-party libraries are not affected. Extend the helper with a
   keyword-only `field_doc` parameter that composes the startup-only
   marker with the original prose so IDE hover documents both *why*
   the field is restart-required and *what* it actually accepts.
   Updated all six restart-required AppConfig fields (`log_level`,
   `database`, `sandbox`, `run_events`, `checkpointer`, `stream_bridge`)
   to pass their original descriptions through the helper.

Tests: two new cases in `test_reload_boundary.py` pin (a) the helper
composition and (b) every AppConfig restart-required field still
surfaces a recognisable substring of its original documentation.

Closes #3289. Adds a unified middleware that enforces per-result budgets
on ALL tool outputs (MCP, sandbox, community, custom), preventing
oversized external tool results from blowing the model context window.

Design informed by claude-code (persistToolResult), hermes-agent
(tool_result_storage), and pi (OutputAccumulator) — the three most
mature implementations in production coding-agent frameworks.

Key features:
- Disk externalization: oversized outputs written to thread-local
  .tool-results/ directory, replaced with compact preview + file
  reference. Model can read full output via read_file with offset/limit.
- Fallback truncation: head+tail truncation when disk is unavailable
  (no thread_data, write failure), ensuring the context is always
  protected.
- read_file exemption: prevents persist-read-persist infinite loops
  (independently discovered by claude-code, hermes-agent, and pi).
- Per-tool threshold overrides via config.
- Line-boundary-aware truncation (no partial lines in previews).
- Multimodal content passthrough (images/structured blocks skip budget).
- Historical ToolMessage patching in wrap_model_call for checkpoint
  recovery scenarios.

Related: #3222 (design RFC), #1844 (comprehensive context management),
#3137 (write_file args compaction), #1677 (sandbox tool truncation).

* test: add MCP content_and_artifact format coverage

Add 5 tests for MCP tool output format (list of content blocks):
- text content blocks are extracted and budgeted
- multiple text blocks are joined and budgeted
- image content blocks are skipped (multimodal passthrough)
- mixed text+image blocks are skipped
- small text blocks pass through unchanged

Total test count: 59 (was 54).

* fix(agent): address Codex review findings for ToolOutputBudgetMiddleware

Three issues identified by Codex code review, all fixed:

1. `enabled` config field was unused — middleware now checks
   `config.enabled` and skips all processing when disabled.

2. `_build_fallback` could exceed `fallback_max_chars` — the marker
   text itself (~139 chars) was not deducted from the budget. Now
   pre-computes marker overhead and falls back to hard slice when
   max_chars is smaller than the marker.

3. Sync file I/O in async path — `awrap_tool_call` now delegates
   `_patch_result` to `asyncio.to_thread` to avoid blocking the
   event loop during disk writes.

Tests updated to use realistic fallback_max_chars values (500+)
that can accommodate the marker overhead, plus two new tests:
- `test_result_never_exceeds_max_chars` (parametric across sizes)
- `test_very_small_max_chars_does_not_crash`

* fix(agent): address Copilot review — path traversal, async perf, shared config

1. Path traversal defense: sanitize tool_name via _sanitize_tool_name()
   (strips separators, .., absolute paths), validate storage_subdir is
   relative, and verify resolved filepath stays inside storage_dir.

2. Async hot-path optimization: add _needs_budget() cheap check before
   asyncio.to_thread offload — small outputs (99% of calls) skip the
   thread overhead entirely.

3. Replace shared module-level _DEFAULT_CONFIG with _default_config()
   factory to prevent cross-instance mutation of mutable fields.

12 new tests: TestSanitizeToolName (5), TestExternalizePathTraversal (3),
TestNeedsBudget (4).

* fix(agent): correct preview hint to match read_file actual API

read_file uses start_line/end_line (1-indexed line numbers), not
offset/limit. The previous wording was copied from hermes-agent
which has a different read_file interface.

* perf(agent): hoist hot-path imports, add model-call pre-scan (review #3303)

Address maintainer review feedback:

1. Hoist inline imports to module level — `import asyncio` (was in
   awrap_tool_call hot path) and `from dataclasses import replace`
   (was in _patch_result) now live at module top.

2. Add a cheap pre-scan to _patch_model_messages so the historical
   message list is not rebuilt on every model call when nothing is
   oversized (the common case once results are budgeted at tool-call
   time). Also adds the same _needs_budget gate to the sync
   wrap_tool_call for symmetry with awrap_tool_call.

The pre-scan is refactored into per-tool-aware helpers
(_effective_trigger / _tool_message_over_budget) that mirror the exact
trigger conditions in _budget_content — including tool_overrides — so
the fast-path can never produce a false negative (silently skipping
budgeting for a tool with a low per-tool threshold).

7 new regression tests lock the per-tool-override-through-pre-scan path
and the model-call early return.

When a provider stops generation for safety reasons (OpenAI/Moonshot
finish_reason=content_filter, Anthropic stop_reason=refusal, Gemini
finish_reason=SAFETY/BLOCKLIST/PROHIBITED_CONTENT/SPII/RECITATION/
IMAGE_SAFETY/...), the response may still carry truncated tool_calls.
LangChain's tool router treats any non-empty tool_calls as executable,
so partial arguments (e.g. write_file with a half-finished markdown)
get dispatched and the agent loops on retry.

Add SafetyFinishReasonMiddleware at after_model: detect safety
termination via a pluggable detector registry, clear both structured
tool_calls and raw additional_kwargs.tool_calls / function_call,
preserve response_metadata.finish_reason for downstream observers,
stamp additional_kwargs.safety_termination for traces, append a
user-facing explanation to message content (list-aware for thinking
blocks), and emit a safety_termination custom stream event so SSE
consumers can reconcile any "tool starting..." UI.

Default detectors cover OpenAI-compatible content_filter, Anthropic
refusal, and Gemini safety enums (text + image). Custom providers are
added via reflection (same pattern as guardrails). Wired into both
lead-agent and subagent runtimes.

RuntimeFeatures, mirroring the existing pattern used by `sandbox`,
`memory`, and `vision`. SDK users can now disable LoopDetectionMiddleware

LoopDetectionConfig — e.g.
`LoopDetectionMiddleware.from_config(my_cfg)` — instead of being stuck

Address two PR review findings:

1. _create_summarization_middleware passed the original (possibly None)
   app_config into create_chat_model, forcing the model factory back to
   ambient get_app_config() and risking config drift between the
   middleware's resolved view and the model's view. Pass the resolved
   AppConfig instance through end-to-end.

2. get_available_subagent_names accepted Any-typed config and forwarded
   it to is_host_bash_allowed, which reads ``.sandbox``. A
   SubagentsAppConfig (also accepted upstream as a sum-type input) has
   no ``.sandbox`` attribute and would be silently treated as "no
   sandbox configured", incorrectly disabling the bash subagent. Guard
   on hasattr and fall back to ambient lookup otherwise.

Adds regression tests for both paths.

* chore: simplify hasattr guard and tighten regression tests

- Collapse if/else into ternary in get_available_subagent_names; hasattr(None, ...) is False so the explicit None check was redundant.
- Drop comments that narrate the change rather than explain non-obvious WHY (test names already convey intent).

Centralize log level parsing in `logging_level_from_config()` and
application in `apply_logging_level()` within `deerflow.config.app_config`.

- Gateway lifespan applies configured log level on startup
- `debug.py` uses shared helpers instead of local duplicates
- `apply_logging_level()` targets only `deerflow`/`app` logger hierarchies
  so third-party library verbosity is not affected; root handler levels
  are only lowered (never raised) to allow configured loggers through
  without suppressing third-party output; root logger level is not modified
- Config field description updated to clarify scope
- Tests save/restore global logging state to avoid test pollution

by _build_acp_section(), which checks get_acp_agents() and returns an
empty string when no ACP agents are configured. This avoids polluting

- Add `Paths.acp_workspace_dir(thread_id)` for per-thread paths

The AioSandboxProvider was not mounting the ACP workspace into the
sandbox container, so /mnt/acp-workspace was inaccessible when the lead
agent tried to read ACP results in docker mode.

Changes:
- `ensure_thread_dirs`: also create `acp-workspace/` (chmod 0o777) so
  the directory exists before the sandbox container starts — required
  for Docker volume mounts
- `_get_thread_mounts`: add read-only `/mnt/acp-workspace` mount using
  the per-thread host path (`host_paths.acp_workspace_dir(thread_id)`)
- Update stale CLAUDE.md description (was "fixed global workspace")

Tests: `test_aio_sandbox_provider.py` (4 new tests)

feat(guardrails): add pre-tool-call authorization middleware with pluggable providers (#1240)
Add GuardrailMiddleware that evaluates every tool call before execution.
Three provider options: built-in AllowlistProvider (zero deps), OAP passport
providers (open standard), or custom providers loaded by class path.

- GuardrailProvider protocol with GuardrailRequest/Decision dataclasses
- GuardrailMiddleware (AgentMiddleware, position 5 in chain)
- AllowlistProvider for simple deny/allow by tool name
- GuardrailsConfig (Pydantic singleton, loaded from config.yaml)
- 25 tests covering allow/deny, fail-closed/open, async, GraphBubbleUp
- Comprehensive docs at backend/docs/GUARDRAILS.md
