fix(summarization): summarize with the run model, fall back on summary-provider failure (#4361)
* fix(summarization): own the run model for compaction; bound failure With summarization.model_name: null the summary model resolved to config.models[0] while the executing model is selected per run; when they differ and models[0]'s provider is broken (expired key, quota, outage) compaction silently failed every triggered turn and context grew unbounded until the main provider 400s the run (#3103's shape), even though the run's own model was healthy. Model ownership is now sourced from the builders, not re-derived at runtime: - The lead, subagent, and manual /compact builders each pass the resolved run model into create_summarization_middleware(run_model_name=...). The middleware no longer reads runtime.context / get_config(), which do not carry a custom agent's or a subagent's resolved model, so a custom-agent lead run and a distinct-model subagent now summarize with their own model, not models[0] / the parent's. Runtime re-resolution and the per-name model cache are removed. - model_name: null summarizes with the run's own model; an explicitly configured summary model generates and falls back to the run model on failure. The fallback is built lazily after the primary fails and its construction is guarded, so a broken fallback cannot skip a healthy primary or escape the automatic failure boundary. Failure is bounded and side-effect-safe: - An empty or whitespace-only response is treated as a generation failure, not a valid summary, so compaction never removes all history for an empty replacement. - compact_state/acompact_state take raise_on_failure independent of force: the manual /compact path always surfaces a generation failure (even force=false) and routes it to the existing ContextCompactionFailed path (HTTP 500 -> frontend error toast) instead of an unconsumed response reason. The automatic path leaves compaction state unchanged. - before_summarization hooks fire only after a replacement summary exists. SummarizationConfig.model_name, config.example.yaml, and docs/summarization.md document the final lead/subagent/manual ownership rules. Part of RFC #4346 (section A). Evaluating fraction/triggers against the run model's profile (profile ownership) is a separate follow-up. * fix(summarization): manual /compact model ownership + fail-open construct/parse Manual /compact carried only agent_name, so it derived the run model from the custom-agent model or config.models[0] and missed the request-selected model the run path uses (request -> custom-agent -> default). Carry model_name through ThreadCompactRequest and the frontend compact call, resolve with the same precedence, and move the custom-agent config read off the event loop (asyncio .to_thread) with user_id so the strict blocking-IO gate is not bypassed by the broad except. Make one summary attempt own its full lifecycle so the fail-open boundary covers construction and response parsing, not just invocation: build each candidate model lazily and guarded (a raising constructor falls through to the healthy run model instead of breaking agent construction), build the model_name:null primary from the run model rather than config.models[0], and run response text extraction inside the invocation try so a failing .text accessor falls back instead of escaping compaction. Adds factory-level constructor-failure, response-extraction-failure (sync/async), and route-path model-ownership tests. |

When before_agent() returns None on empty turns, the LastValue
uploaded_files field retains the previous turn's filenames.
list_uploaded_files then incorrectly excludes those files as
"current-run" files, making them invisible until the next upload.
Fix: return {"uploaded_files": []} instead of None to explicitly
clear state. Add two-turn regression test covering the exact
scenario from review feedback.

The lazy-loading PR renamed <uploaded_files> to <current_uploads>.
The anti-drift guard scans all framework XML blocks and requires each
to be either blocked or explicitly exempted. current_uploads wraps
trusted server-generated file metadata, not user input, so it belongs
in the exempt set.

When _extract_text_from_content and message_content_to_text disagree on
multimodal list content and rfind fails, use content[0] (server-injected
<current_uploads> block) vs content[1:] (user blocks) to sanitize only
user blocks. Raw strings and non-standard dict blocks that
_extract_text_from_content misses are now also sanitized.
Non-distinguishable paths (< 2 text blocks, non-list content) still
degrade to full sanitization (safe — server block may be escaped but
user forgery never leaks). All fallback paths log via logger.warning.

_format_uploaded_files_block() function. UploadsMiddleware is the
sole upload-context producer for both IM and web paths.

fix(subagents): inherit summarization middleware and harden step capture (#3875 Phase 3) (#4009)
Phase 3 of #3875 — subagents previously inherited none of the lead's context-compaction, so a deep-research subagent (max_turns up to 150) could accumulate >1M cumulative input before max_turns/timeout/token_budget engaged, even after Phase 2's budget capped the pathological tail. - Gate the subagent runtime chain on the SAME ``app_config.summarization.enabled`` switch the lead reads (per maintainer guidance in #3875), via the shared ``create_summarization_middleware`` factory. One config covers both chains; no separate ``subagents.summarization`` field. No-op when summarization is off (factory returns None). - ``skip_memory_flush=True`` on the subagent path: the factory otherwise attaches ``memory_flush_hook`` (when memory.enabled), which flushes pre-compaction messages into durable memory keyed by thread_id. Subagents share the parent's thread_id, so without skipping the hook a subagent's internal turns would pollute the PARENT thread's durable memory (#3875 Phase 3 review point). - Harden ``capture_new_step_messages`` to tolerate history contraction: summarization rewrites the messages channel via ``RemoveMessage(id=REMOVE_ALL_MESSAGES)``, shrinking len(messages) below the step-capture cursor. Without a reset, every step appended after the compaction point was dropped until length overtook the stale cursor (#3845 interaction, maintainer validation point (a)). Cursor now resets to the new tail; id/content dedup prevents re-emitting pre-compaction steps. - Couple the DEFAULT token-budget ceiling to ``summarization.enabled`` (#3875 Phase 3 review point): 1M when compaction is on, 2M when off (preserves Phase 2's deliberate headroom for summarization-off deep-research runs that can exceed 1M). A user-set budget (global or per-agent) always wins regardless of the switch. Flagged tunable. * feat(mcp): auto-promote deferred MCP tools from routing hints
When tool_search.enabled=true defers MCP tool schemas, PR1 routing hints
still require the model to spend a tool_search discovery round trip before
it can call the tool the routing metadata already points at. This adds a
McpRoutingMiddleware that matches the latest user message against PR1
routing keywords and promotes the matching deferred schemas before the
model call, removing that round trip.
Design (soft routing, opt-in, additive):
- Matches only the latest real HumanMessage (shared is_real_user_message
helper, reused by SkillActivationMiddleware so the two cannot drift);
case-insensitive substring match, no tokenizer dependency.
- Ordering: priority desc, then tool name asc; capped by the new global
tool_search.auto_promote_top_k (default 3, clamped 1..5). Does not add or
consume a per-tool auto_promote_top_k (PR1 schema unchanged); a per-tool
value is ignored with a DEBUG note.
- Returns a plain {"promoted": ...} state update (not a Command) and relies
on ThreadState.merge_promoted for union/dedupe, so auto-promote and a
model-triggered tool_search converge on the same catalog hash.
- Installed before DeferredToolFilterMiddleware on every deferred-tool path
(lead agent, subagent, embedded client, webhook via shared builders);
a construction-time assert rejects the reversed order. catalog_hash is
None / no routing index is a complete no-op, so bootstrap and ACP skip it.
- Privacy: never executes tools, never promotes policy-filtered tools, adds
no routing keywords or matched tool names to trace metadata or INFO/WARN
logs.
No behavior change when tool_search.enabled=false.
Tests: index construction, matching semantics, middleware state updates,
same-cycle deferred-filter interaction, lead/subagent/embedded-client
builder wiring + order invariant, config clamping, config.example.yaml
parseability, and privacy assertions.
* refactor(mcp): address auto-promote review nits
- executor: access app_config.tool_search.auto_promote_top_k directly to match
the lead-agent and embedded-client paths (drop the over-defensive getattr that
masked missing config); update the subagent test mock to carry tool_search.
- tool_search / mcp_routing_middleware: cross-reference the duplicated routing
priority/keyword normalization between the builder and the middleware's
defensive _normalize_index so they cannot silently drift.
- MCP_SERVER.md: document that auto-promote keyword matching is a case-insensitive
substring test (not word-boundary), advising distinctive keywords.

* fix(security): neutralize prompt-injection tags in remote tool results
User input is already neutralized for framework/injection tags, but tool
results are not. Remote content fetched by web_fetch/web_search is equally
untrusted and can carry a forged <system-reminder> block that reaches the
model verbatim as authoritative context.
Extract a shared neutralize_untrusted_tags() primitive from
InputSanitizationMiddleware and apply it to remote-content tool results
(web_fetch/web_search/image_search) via a new ToolResultSanitizationMiddleware.
Local tool output (bash/read_file) is left untouched so legitimate code/file
content is never mangled.
* test: update subagent middleware count for tool-result sanitizer
The new ToolResultSanitizationMiddleware adds one entry to the shared runtime
chain (11 -> 12). Update the subagent count assertion, use a lazy import for
neutralize_untrusted_tags so the module loads even when tests stub the
input-sanitization module, and document the new middleware in AGENTS.md.
* fix(security): address review — sanitize bare str list items; document MCP scope
- Neutralize bare str elements inside a ToolMessage content list (previously
only {type:text} dict blocks were rewritten), matching the str-in-list shape
ToolOutputBudgetMiddleware._message_text already anticipates.
- Document the name-based allowlist limitation: MCP remote-content tools
registered under arbitrary names (e.g. fetch_url) are not covered; a name
heuristic is avoided to prevent mangling local tool output, with metadata
tagging tracked as a follow-up. Add a regression test pinning this boundary.

fix(subagents): unify guardrail caps on additive stop_reason + add token_budget (#3875 Phase 2) (#3980)
Phase 2 of #3875. Two guardrail axes can end a subagent run early — the turn budget (GraphRecursionError) and the token budget (TokenBudgetMiddleware) — and both now surface *why* through one additive `subagent_stop_reason` field instead of a status enum. This completes and course-corrects Phase 1 (#3949), which shipped the turn-budget cap as a `max_turns_reached` status enum. The agreed Phase 2 design replaces that enum with an optional `stop_reason` field (token_capped | turn_capped | loop_capped): a new enum value would break v1 consumers, while an additive field is ignored by older frontends and ledger readers. `max_turns_reached` and SubagentStatus.MAX_TURNS_REACHED are removed. - subagents.token_budget config (default enabled, 2,000,000 tokens, warn 0.7) with per-agent override; TokenBudgetMiddleware is now attached in build_subagent_runtime_middlewares so the cost-ceiling backstop engages for every subagent. The hard-stop does not raise — it strips tool_calls and lets the run finish with a final answer, recording the cap on a per-run consume_stop_reason() accessor. - executor.py: on normal completion it reads consume_stop_reason() and stamps completed + token_capped when the budget fired; on GraphRecursionError it recovers the last AIMessage partial (completed + turn_capped) or, if nothing usable survived, failed + turn_capped. SubagentResult gains stop_reason. - status_contract.py / contracts/subagent_status_contract.json (v2) / frontend subtask-result.ts: additive subagent_stop_reason field, pinned by test_status_values_match_contract / test_stop_reason_values_match_contract. - task_tool.py + delegation_ledger.py: drop the max_turns_reached paths; the ledger captures stop_reason and renders model-facing "capped" guidance so the lead reuses a capped completion knowingly. The 2,000,000-token default is deliberately loose (tighten to taste) — it would have roughly halved the reported 4.4M burn while leaving legitimate deep-research runs (max_turns=150) room. Subagent summarization is a follow-up. |

Non-recoverable (rate_limited, transient) still escalate WARNED → BLOCKED

fix(subagents): inherit LoopDetectionMiddleware to break tool loops (#3875) (#3931)
Subagents build their runtime chain via build_subagent_runtime_middlewares, which attached none of the lead's runaway guards — no LoopDetectionMiddleware. A degenerate subagent tool loop therefore ran unchecked until max_turns, re-sending a growing context each turn (the #3875 4.4M-token burn). Append LoopDetectionMiddleware.from_config(app_config.loop_detection) to the subagent chain, gated by the existing loop_detection.enabled config (default on) — no new config field. Registered before SafetyFinishReasonMiddleware to match the lead's after-model ordering (the safety middleware requires placement after LoopDetection). Subagents disallow task, so only the tool-loop heuristic can fire here — no recursive-delegation false positives to worry about. Phase 1 of #3875 (smallest blast radius). Phase 2 adds a deterministic turn/token budget with a lead-visible stop reason; Phase 3 defers summarization. refactor(middlewares): declarative layered builder; pin chain order with tests (#3809)
Split `_build_runtime_middlewares`'s flat list into three named declarative sublists (outer_wrappers / thread_hooks / tail) and drop the `middlewares.insert(2, UploadsMiddleware())` magic-index pattern. The declarative structure makes the layering self-documenting and immune to position drift when the head of the list changes. Move UploadsMiddleware to run after ThreadDataMiddleware in the chain. Under the previous order (a magic-index artifact introduced when #3662 prepended InputSanitizationMiddleware), UploadsMiddleware scanned the uploads directory before ThreadDataMiddleware created it under lazy_init=False, so historical files could be missed on the first run of a thread. Narrow path — the upload endpoint normally pre-creates the directory — but the order is the correct semantic and is now locked. Documentation: - backend/AGENTS.md middleware chain renumbered: ThreadDataMiddleware is now #3, UploadsMiddleware #4 (was reversed). Tests (backend/tests/test_tool_error_handling_middleware.py): - test_build_lead_runtime_middlewares_orders_thread_data_before_uploads — focused td_idx < um_idx assertion. - test_build_lead_runtime_middlewares_chain_order_matches_agents_md — full-chain order pin using real classes, so a swap between any pair is caught (the existing FakeMiddleware-stubbed tests cannot detect this). - test_lead_runtime_chain_finds_historical_uploads_under_lazy_init_false — integration anchor: under lazy_init=False, ThreadDataMiddleware creates the dirs, then UploadsMiddleware surfaces a pre-existing historical file in the injected <uploaded_files> context. |

* feat(subagents): extend deferred MCP tool loading to subagents (#3341) Subagents now reuse the lead agent's deferred-tool path: when tool_search.enabled, MCP tool schemas are withheld from the model and surfaced by name in <available-deferred-tools>, fetched on demand via the generated tool_search helper. DeferredToolFilterMiddleware deterministically rewrites request.tools to hide the deferred schemas (the prompt section is discovery only, not enforcement). Consolidates the assembly into deerflow.tools.builtins.tool_search, now the single home for both assemble_deferred_tools (centralized fail-closed guard, replacing the lead-only private _assemble_deferred) and the relocated get_deferred_tools_prompt_section. Shared by every build path: lead agent, embedded client, and subagent executor. tool_search is appended after the subagent's name-level tool policy and is treated as infrastructure: its catalog is built from the already policy-filtered list, so it can never surface a tool the policy denied. Follow-up to #3370. Fixes #3341. * test(subagents): assert the real middleware builder emits a working deferred filter (#3341) The existing recipe test hand-constructs DeferredToolFilterMiddleware, so it cannot catch a regression in how build_subagent_runtime_middlewares (the call executor._create_agent actually makes) wires the deferred setup into the filter. Add a test that sources the filter from the real builder given a real setup and runs it through a graph: a wrong catalog hash would silently stop promotion, a dropped filter would stop hiding — both now caught. Adds a unified middleware that enforces per-result budgets on ALL tool outputs (MCP, sandbox, community, custom), preventing oversized external tool results from blowing the model context window. Design informed by claude-code (persistToolResult), hermes-agent (tool_result_storage), and pi (OutputAccumulator) — the three most mature implementations in production coding-agent frameworks. Key features: - Disk externalization: oversized outputs written to thread-local .tool-results/ directory, replaced with compact preview + file reference. Model can read full output via read_file with offset/limit. - Fallback truncation: head+tail truncation when disk is unavailable (no thread_data, write failure), ensuring the context is always protected. - read_file exemption: prevents persist-read-persist infinite loops (independently discovered by claude-code, hermes-agent, and pi). - Per-tool threshold overrides via config. - Line-boundary-aware truncation (no partial lines in previews). - Multimodal content passthrough (images/structured blocks skip budget). - Historical ToolMessage patching in wrap_model_call for checkpoint recovery scenarios. Tests updated to use realistic fallback_max_chars values (500+) that can accommodate the marker overhead, plus two new tests: - `test_result_never_exceeds_max_chars` (parametric across sizes) - `test_very_small_max_chars_does_not_crash` * fix(agent): address Copilot review — path traversal, async perf, shared config 1. Path traversal defense: sanitize tool_name via _sanitize_tool_name() (strips separators, .., absolute paths), validate storage_subdir is relative, and verify resolved filepath stays inside storage_dir. 2. Async hot-path optimization: add _needs_budget() cheap check before asyncio.to_thread offload — small outputs (99% of calls) skip the thread overhead entirely. 3. Add a cheap pre-scan to _patch_model_messages so the historical message list is not rebuilt on every model call when nothing is oversized (the common case once results are budgeted at tool-call time). * fix(runtime): suppress tool execution when provider safety-terminates with tool_calls When a provider stops generation for safety reasons (OpenAI/Moonshot finish_reason=content_filter, Anthropic stop_reason=refusal, Gemini finish_reason=SAFETY/BLOCKLIST/PROHIBITED_CONTENT/SPII/RECITATION/ IMAGE_SAFETY/...), the response may still carry truncated tool_calls. LangChain's tool router treats any non-empty tool_calls as executable, so partial arguments (e.g. write_file with a half-finished markdown) get dispatched and the agent loops on retry. Add SafetyFinishReasonMiddleware at after_model: detect safety termination via a pluggable detector registry, clear both structured tool_calls and raw additional_kwargs.tool_calls / function_call, preserve response_metadata.finish_reason for downstream observers, stamp additional_kwargs.safety_termination for traces, append a user-facing explanation to message content (list-aware for thinking blocks), and emit a safety_termination custom stream event so SSE consumers can reconcile any "tool starting..." UI. Default detectors cover OpenAI-compatible content_filter, Anthropic refusal, and Gemini safety enums (text + image). Custom providers are added via reflection (same pattern as guardrails). Wired into both lead-agent and subagent runtimes. Skip the eager resolve in __init__ when it would require loading the config file, and defer to _create_agent (which already has the app_config or get_app_config() fallback). |

* refactor: thread app_config through middleware factories Continues the incremental config-refactor sequence (#2611 root, #2612 lead path) one layer deeper into the middleware factories. Two ambient lookups inside _build_runtime_middlewares are eliminated and the LLMErrorHandling band-aid removed: - _build_runtime_middlewares / build_lead_runtime_middlewares / build_subagent_runtime_middlewares now require app_config: AppConfig. - get_guardrails_config() inside the factory is replaced with app_config.guardrails (semantically identical — same default-factory GuardrailsConfig — verified by direct equality check). - LLMErrorHandlingMiddleware.__init__ now requires app_config and reads circuit_breaker fields directly. The class-level circuit_failure_threshold / circuit_recovery_timeout_sec defaults are removed along with the try/except (FileNotFoundError, RuntimeError): pass band-aid — the let-it-crash invariant the rest of the refactor enforces. Caller chain (already-resolved app_config sources): - _build_middlewares in lead_agent/agent.py: reorder so resolved_app_config = app_config or get_app_config() is computed BEFORE build_lead_runtime_middlewares is called, then passed as kwarg. - SubagentExecutor: optional app_config parameter (mirrors the lead-agent pattern); _create_agent does the same `or get_app_config()` fallback at agent-build time, so task_tool callers don't need to plumb app_config through yet (typed-context plumbing for tool runtimes is a separate refactor). Tests: - test_llm_error_handling_middleware: _make_app_config helper using AppConfig(sandbox=SandboxConfig(use="test")) — same minimal-config pattern conftest already uses. Three direct LLMErrorHandlingMiddleware() calls each followed by post-construction circuit_breaker mutation fold cleanly into _build_middleware(circuit_failure_threshold=..., circuit_recovery_timeout_sec=...). Verification: - tests/test_llm_error_handling_middleware.py — 14 passed - tests/test_subagent_executor.py — 28 passed - tests/test_tool_error_handling_middleware.py — 6 passed - tests/test_task_tool_core_logic.py — 18 passed (verifies task_tool unchanged behavior) - Full suite: 2697 passed, 3 skipped.
