docs(backend): point the stream-mode translation anchor at stream_modes.py (#6021)

* docs(backend): point the stream-mode translation anchor at stream_modes.py

* docs(backend): use one anchor spelling within the stream-mode table row

The row named the translator by its backend-relative path and its call
site by the harness-relative short form, so the same symbol was written
two ways two rows apart.
