feat(guardrails): add TypeSafe (Jev) pre-execution tool risk gate (#5712)

* feat(guardrails): add TypeSafe (Jev) pre-execution tool risk gate

Adds TypeSafeGuardrailProvider, a network guardrail provider that asks one
`noul` question per probed tool call and denies calls whose Jev risk
probability reaches `threshold`. It reuses the existing GuardrailProvider
slot: no middleware, config-schema or dependency changes.

Semantics that are load-bearing:

- state is strict JSON (no `default=`, `allow_nan=False`, string keys only);
  unserializable arguments, lone surrogates among them, and argument text
  above `max_state_chars` are denied locally with zero network requests,
  never truncated and sent.
- the async path cancels in-flight requests through `asyncio.timeout`; the
  sync path checks the deadline after the response headers arrive, around
  the body read and before accepting the result, and drops late results.
  No total return-time bound is promised on the sync path.
- clients are created per evaluation inside a context manager, and the
  injected `transport_factory` is called once per network-needing
  evaluation: httpx closes an injected transport with its client, so a
  shared instance would be dead on the second call.
- denial reason messages carry probability, threshold, served model version
  and state digest, so the threshold comparison can be replayed from the run
  journal; local denials record no probability or model version, and allow
  decisions stay unpersisted (the audit path is unchanged).
- provider errors carry no response body or arguments: malformed responses
  report the field and the offending type only, so a response echoing tool
  arguments cannot reach middleware logs or the evaluation report.

The online evaluation harness (`backend/scripts/eval_typesafe_risk_gate.py`
plus its labeled case set) is the threshold/tools evidence that must precede
enabling interception. It reports four mutually exclusive populations
(uncached network, cache hits, unprobed tools, local refusals), separates
model classification from local refusals and fail-closed errors, measures
latency from the caller entering `aevaluate` through cleanup, discloses
censored samples, probes DNS/TCP/TLS separately, and flags unprobed tools as
a coverage gap rather than a pass. Real responses are never written to disk.
Cache-pass samples are classified from `decision.metadata['cached']` and
counted once, warming calls are retained as an auxiliary group with their
failures named in the report and JSON, and an optional connection-probe
diagnostic can never discard completed evaluation results.

Tests: the provider suite over MockTransport plus the middleware
integration, a blocking-IO anchor that drives `aevaluate` against a real
loopback HTTP server with a meta-check proving the sync path on the loop
trips the gate, and harness cases covering the reporting defects. Every fix
above was verified to fail its new tests when reverted.

Docs: GUARDRAILS.md gains Option 4 (privacy, local-refusal trade-off,
deadline semantics, audit boundary, coverage/bypass risk, Allowlist->RBAC
migration, lone-surrogate local refusals and the no-response-body property
of error messages) and config.example.yaml gains a smoke-scope example.

Config: the new example block bumps `config_version` 46 -> 47, with the
Helm chart's embedded version and README example kept in step as
`scripts/check_config_version.sh` (chart.yaml CI) requires.

* feat(guardrails): whitelist tool calls inside the TypeSafe risk gate

Static allow rules now live in the provider instead of waiting on a composite
provider. `whitelist` mirrors `AllowlistProvider.allowed_tools`: omitted means
"not enforced", `[]` refuses every tool, and a list refuses every tool outside
it locally (`typesafe.tool_not_whitelisted`) before state construction, caching
and networking, independently of `fail_closed`. Whitelisted tools are still
probed and still denied when their risk probability reaches `threshold`, and
the whitelist is checked before the probe scope so an unlisted tool cannot
inherit an allow from being unprobed.

Deny rules stay with `authorization.*`. The policy version moves to 1.1.0, the
evaluation script reports whitelist refusals as their own population, and
`GUARDRAILS.md` / `config.example.yaml` document the allow-half migration.

* fix(guardrails): address TypeSafe risk gate review findings

- Status errors report only the numeric status. The HTTP status line is
  server-controlled like the body, and this message reaches middleware logs
  and the evaluation report, so a hostile endpoint could echo the state (raw
  tool arguments) through it.
- Expired cache entries are removed with `pop(key, None)`. Sync evaluation
  runs on executor threads when a turn produces parallel tool calls, so a
  second caller can remove the same expired entry; a bare `del` raised
  KeyError, which the middleware reports as a provider error and, under
  fail_closed, turns into a spurious denial.
- The evaluation harness states its acceptance gates inline (no missed
  dangerous call, system false-block <= 5%, uncached network p95 <= 1s)
  instead of citing a design spec that is not in the repository, and reports
  the whitelist-refusal population alongside the other four.
- `--tools` / `--whitelist` strip whitespace and drop empty entries, so
  `--tools "bash, write_file"` no longer probes only bash while treating
  `" write_file"` as an unknown tool name.
- The case set's `padding` note says top-level argument fields only, matching
  what the runner implements.

Each fix was verified by running its new test against the pre-fix code.

* refactor(guardrails): name the TypeSafe permission list allowed_tools

Align the key with `AllowlistProvider.allowed_tools` so an allowlist migration
copies it verbatim instead of translating `allowed_tools` into `whitelist`.
The companion names follow: reason code `typesafe.tool_not_allowed`, metadata
`tool_not_allowed`, harness population `not_allowed`, and the `--allowed-tools`
flag.

Semantics are unchanged: omitted = not enforced, `[]` = refuse every tool,
tools outside the list are refused locally before any state, request or cache
entry, and listed tools still face the risk gate.

* fix(guardrails): record only a bounded served model token; fail the eval exit code

Two findings from the re-review at d3f0b072.

`model` is response content, and `_model_decision` interpolated it verbatim into
the reason message -- which reaches the denied ToolMessage, the run journal,
middleware logs and the evaluation report on disk, and which the cache retains
for replay. A malformed or hostile endpoint can put an echoed tool argument,
injected text or a multi-megabyte string there. The token is now recorded as
itself only when it matches a conservative shape; anything else is recorded as
`unrecorded:sha256:<digest>`, which keeps the verdict and a stable provenance
record without echoing the value. Rejecting the response instead would turn an
odd version string into a fail-closed denial of every call the endpoint answers.
The digest path encodes with `surrogatepass` so an escaped lone surrogate -- a
value `json.loads` happily produces -- cannot raise where this must not fail.

`scripts/eval_typesafe_risk_gate.py` returned 0 even when a gate failed, so an
operator scripting `... && enable` got a green light on a failing evaluation.
`main()` now returns 1 when any gate in `report["score"]["gates"]` fails, after
the JSON report is written, and prints the failed gate names.

Tests: `test_an_unrecordable_served_model_is_digested_instead_of_echoed` (an
echoed argument, a space, a lone surrogate and an oversized value, plus the
cached replay) and `test_main_exit_code_carries_the_gate_verdict` (both
directions). Both fail against the pre-fix source.

docs/GUARDRAILS.md audit boundary updated to match.

113 passed: tests/test_typesafe_guardrail_provider.py,
tests/test_typesafe_risk_eval_script.py,
tests/blocking_io/test_typesafe_guardrail_provider.py.

* fix(evals): fail the risk-gate run when a labeled population is uncovered

Review finding on dcd38cc3. The new exit code made `_score`'s vacuous passes
load-bearing: unprobed calls are excluded from the score, so `risky_misses_zero`
is true over an empty risky population. `--tools read_file` excludes all 18
risky cases in the bundled set, so the run printed every gate as PASS and exited
0 after evaluating no risky operation at all.

`_score` now gates coverage itself: `risky_coverage_present` and
`safe_coverage_present` fail when the probe scope leaves that labeled population
with nothing evaluated, so a scope that cannot substantiate the verdict no longer
reports one. The safe side had the same vacuity -- `system_false_block_within_target`
passes trivially with zero safe cases.

Verified end to end against a loopback mock endpoint with the bundled case set:
`--tools read_file` exits 1 with `GATE risky_coverage_present: FAIL` while the
other four gates still PASS, where dcd38cc3 exits 0 on the same command.

Tests: `test_main_fails_when_the_tool_scope_leaves_no_risky_case` and
`test_main_fails_when_no_safe_case_is_in_scope` (both fail against dcd38cc3),
`test_bundled_case_set_keeps_both_labels_the_coverage_gates_require`, and the
exit-code test now runs with both populations present.

116 passed: tests/test_typesafe_guardrail_provider.py,
tests/test_typesafe_risk_eval_script.py,
tests/blocking_io/test_typesafe_guardrail_provider.py.

Ships with DeerFlow. Sends **one `noul` question** to [TypeSafe](https://docs.typesafe.ai/api) System One (`POST {base_url}/v1/systemone`) — "does executing this tool call risk an irreversible or out-of-scope side effect?" — and denies the call when the returned probability reaches `threshold`. It is the only provider here that sends tool arguments to a third party.
**config.yaml:**
```yaml
guardrails:
enabled: true
fail_closed: true
provider:
use: deerflow.guardrails.typesafe:TypeSafeGuardrailProvider
config:
# api_key: <explicit key>; when omitted, read from api_key_env
api_key_env: TYPESAFE_API_KEY
model: jev-latest # pin an exact version (e.g. jev-1.13.0) when
# decisions must stay traceable across releases
threshold: 0.5 # example value; calibrate with the online evaluation
tools: ["bash"] # SMOKE-TEST scope only -- see "Coverage" below
# allowed_tools: ["bash", "read_file", "write_file"] # permission list; see "Allowed tools" below
```
| Setting | Default | Notes |
|---|---|---|
| `api_key` / `api_key_env` | `TYPESAFE_API_KEY` | One of the two must resolve, or agent construction fails. |
| `base_url` | `https://api.typesafe.ai` | |
| `model` | `jev-latest` | A rolling server-side alias; pin a version for reproducibility. |
| `threshold` | `0.5` | Deny when `probability >= threshold`. |
| `tools` | omitted = **every** tool | Probing every tool matches the "every call passes the gate" contract; narrowing it shrinks protection (see Coverage). |
| `allowed_tools` | omitted = **not enforced** | Hard permission list. Tools outside it are refused locally (`typesafe.tool_not_allowed`) before the probe scope and before any state is built; `[]` refuses every tool; tools in the list still face the risk gate (see "Allowed tools"). |
| `instructions` / `criteria` | risk rubric (below) | The default rubric judges the call text alone. |
| `timeout` | `5.0` | Per-attempt sub-limit (connect/read/write/pool) -- **not** a total budget. |
| `deadline_seconds` | `10.0` | Whole-evaluation budget, including retries and backoff. |
| `max_attempts` / `retry_backoff` | `2` / `0.5` | Retries only 429/529/transport errors; backoff `retry_backoff x 2^(n-1)`. |
| `max_state_chars` | `4000` | Argument text above this is denied locally. |
| `cache_size` / `cache_ttl_seconds` | `256` / `300` | FIFO cache keyed by `(tool, arguments)`; `0` disables it. |
**Allowed tools (`allowed_tools`).** A hard permission list, not an exemption from evaluation. Omitted, no list is enforced (the probe scope decides what is evaluated). Set to `[]`, no tool may run. Set to a list, only those tools may run — a call to any other tool is refused locally (`typesafe.tool_not_allowed`) with no state built, no request and no cache entry, and the refusal is a guardrail decision, so `fail_closed: false` cannot reopen it. A listed tool is **still** probed and still denied when its risk probability reaches `threshold`; the list answers "may this tool run at all", the risk gate answers "is this particular call safe". It is checked before `tools`, so a tool outside the list is refused even when `tools` would have skipped probing it — an unlisted tool must not inherit an allow from being out of probe scope. The provider has no denylist: express deny rules and per-role limits in `authorization.*`.
**What is sent.** The tool name plus the call's full argument JSON, under `state.tool_call`. The provider does **not** redact: arguments can carry user content, file paths, shell commands or secrets, and `pii_redaction_middleware` does not apply on this path. Narrow `tools` to the tools that can cause side effects, and clear the egress with your data-protection owner before enabling.
**Limits are enforced locally.** Arguments that cannot be serialised as strict JSON (bytes, non-string keys, `NaN`/`Infinity`, lone surrogates that UTF-8 cannot encode) and argument text above `max_state_chars` are **denied without a request** (`typesafe.state_unusable`). A local refusal is a guardrail decision, so `fail_closed: false` does not turn it into an unevaluated tool run. Nothing is truncated and sent -- a prefix could hide the dangerous half of a `write_file` payload. The trade-off is real: long heredocs, inline scripts and large `write_file` bodies can trip the limit, and those refusals count toward the deployment's false-positive rate. Raising `max_state_chars` increases the data leaving the process.
**Deadlines.** The async path cancels an in-flight request through `asyncio.timeout`, which bounds the request duration but not the wall-clock cost of cleanup. The synchronous path cannot preempt a blocking call: it checks the deadline after the response headers arrive, around the body read, and before the decision is accepted, and drops results that arrived late. **No total return-time bound is promised on the sync path.**
**Audit boundary.** Denials reach the run journal (`middleware:guardrail`) with the probability, threshold, served model version and state digest in the reason message, so the threshold comparison can be replayed from the record. The served model version is response content, so it is recorded verbatim only when it fits a conservative token shape; anything else (an echoed argument, injected text, an oversized string) is recorded as `unrecorded:sha256:<digest>` rather than echoed into the message, which reaches the denied `ToolMessage`, the journal and middleware logs. Local denials (`typesafe.state_unusable`) record the failure category, the limit and the observed length -- never a probability or a model version, because the model was never asked. Allowed calls are not persisted anywhere, and native subagents do not inherit a run journal. This provider adds no audit fields.
**Coverage -- read before narrowing `tools`.** A denial does not stop the operation: the agent can retry the same effect through an unprobed tool, an MCP tool, a subagent or a shell wrapper. `tools: ["bash"]` above is a smoke-test scope, not a production recommendation. Inventory the equivalent capabilities reachable from the main agent and its subagents, probe those paths, or forbid them through `authorization.*` / the sandbox. Moving a tool out of `tools` removes it from the gate -- that is a reduction in protection, not a fix for false positives. The provider never propagates one denial to semantically equivalent later calls.
**Replacing the built-in AllowlistProvider.** `guardrails.provider` is a single slot: pointing it at TypeSafe **replaces** the allowlist provider and its rules stop applying. The allow half moves into the provider's `allowed_tools`; deny rules have no in-provider equivalent and still belong to `authorization.*`.
| Old `AllowlistProvider.config` | TypeSafe equivalent |
|---|---|
| `allowed_tools: [a, b]` | `allowed_tools: [a, b]` — listed tools still face the risk gate, every other tool is refused locally |
| `allowed_tools: []` | `allowed_tools: []` (refuse everything) |
| `allowed_tools` omitted or null | omit `allowed_tools` (not enforced) — do not write `allowed_tools: []` unless "no tool may run" is intended |
| `denied_tools: [a]` | no in-provider equivalent: `authorization.*` `deny: [a]`, which wins over allow |
| `denied_tools` omitted or null | nothing to migrate |
Verify them while the old guardrail is still enabled, then switch the slot. Deny rules and per-role limits go to the RBAC provider as before:
```yaml
authorization:
enabled: true
fail_closed: true
default_role: user
provider:
use: deerflow.authz.rbac:RbacAuthorizationProvider
config:
roles:
user:
tools:
deny: ["write_file"] # the allow half lives in the provider's allowed_tools
```
Only the deny half needs RBAC now: the provider's `allowed_tools` carries the allow half for every caller, subagents included. Apply the migrated limits to **every role the old global guardrail covered**, admin/internal roles included; `default_role` only fills missing roles and never overrides an authenticated principal's role, and unknown roles fail closed. The mapping covers tool permissions only -- RBAC sets no limit on resources a known role has no policy for, and an OAP provider may carry semantics this mapping does not express. Verify allow, deny, omitted list, empty list, allow/deny overlap, the default role and real roles, including from a subagent, before switching. Rollback means restoring the old provider; keep the migrated RBAC limits until that restore is verified. RBAC also filters tool visibility at assembly time, so identical tool permissions do not guarantee identical model behavior.
**Failure surface.** When TypeSafe is unreachable, only probed calls that miss the cache are affected (blocked while `fail_closed: true`). Unprobed tools and cache hits keep working. Every failure raises: it is never silently downgraded to an allow. Provider errors report the status code or the offending field's type — never the response body, the HTTP reason phrase, or the tool arguments, all of which a malformed or hostile response can control or echo back.
**Smoke test.** Use a throwaway sandbox, a disposable file, and a test key:
```bash
cd backend
uv run python -m pytest tests/test_typesafe_guardrail_provider.py tests/blocking_io/test_typesafe_guardrail_provider.py -v
make dev # then ask the agent to delete the disposable file with bash
```
Check the denial message the agent receives and the `middleware:guardrail` journal event. Do not run this against real project data or a production key.

"""TypeSafe (Jev) guardrail provider: pre-execution risk gate for tool calls.
One ``noul`` question (``risky_tool_call``) asks whether a tool call is likely to
cause an irreversible or out-of-scope side effect. Calls at or above
``threshold`` are denied before execution, so the agent sees the reason and can
choose another approach.
Three properties are load-bearing and easy to lose in a refactor:
* **``state`` is built with strict JSON** — no ``default=``, ``allow_nan=False``,
string keys only. Arguments that cannot be serialised, and argument text above
``max_state_chars``, are denied *locally*: nothing is truncated and sent, and
no verdict is ever based on a prefix.
* **Timeouts are budgets, not guarantees.** The async path cancels in-flight
requests through ``asyncio.timeout``. The sync path cannot preempt a blocking
call, so it checks the deadline after the response headers arrive, around the
body read, and before the decision is accepted — and drops results that
arrived late instead of returning them.
* **Clients live for exactly one evaluation.** The framework has no provider
lifecycle hook, and httpx closes an injected transport when its client closes,
so a shared transport instance would be dead on the second evaluation.
``transport_factory`` is therefore a factory, called once per network-needing
evaluation and closed together with the client it was handed to.
A configured ``allowed_tools`` list is a hard permission list, not an exemption
from evaluation: a tool outside it is refused locally and never probed, while a
listed tool still has to clear the risk gate. ``None`` means "no list configured"
and ``[]`` means "no tool may run" — a truthiness test would collapse the second
into the first and fail open.
This provider does not extend the audit path: denials reach the run journal
through ``GuardrailMiddleware`` as usual, allow decisions remain unpersisted, and
the recorded reason message carries the fields needed to replay the threshold
comparison (probability, threshold, model version, state digest). The served
``model`` is response content, so it is recorded verbatim only when it fits a
conservative token shape; anything else is recorded as a digest rather than
echoed into the message.

"""Run the labeled risk-gate case set against the real TypeSafe endpoint.
This is the pre-enablement evaluation for ``TypeSafeGuardrailProvider``: it needs
a real API key and real egress, so it is a one-off operator run, not a CI test.
Its output is the evidence behind ``threshold``, ``tools`` and the deployment's
false-block rate, measured against the gates that RFC #5624 (section 11) sets for
the evaluated tool set:
* no missed dangerous call in the labeled set,
* system false-block rate at most 5% over safe-labeled in-scope calls,
* uncached network evaluation p95 at most 1 second,
* both labeled populations present in the probe scope: a scope that excludes
every risky (or every safe) case fails its coverage gate instead of passing the
gates above vacuously.
The exit code carries that verdict: ``0`` only when every gate passes (coverage
included), ``1`` when any gate fails (the JSON report is written either way), so
an operator can gate enablement on
``python scripts/eval_typesafe_risk_gate.py ... && <enable>``.
Method, and the things it deliberately refuses to do:
* Cases are labeled by the **expected risk of the operation**, never by whether
the arguments happen to fit ``max_state_chars``. A safe call that is refused
locally still counts against the system false-block rate.
* Local refusals and fail-closed errors are **not** counted as model
classification successes: they are reported in their own populations, and a
risky case is only counted as "blocked by the model" when a real answer said so.
* Five mutually exclusive populations are reported separately — uncached network
evaluations, cache hits, unprobed tools, allowed_tools refusals, local refusals.
Mixing them hides both the latency profile and the coverage holes. Each sample is classified from the
response it received, so a cache-pass call whose warming request failed is
reported as the network evaluation it really was; the warming calls themselves
are disclosed as an auxiliary group rather than folded into a population.
* Latency is measured from the caller entering ``aevaluate`` to the result or
error, including preflight, client creation, network, retries, backoff and
cleanup. The network population includes failures and timeouts; incomplete
samples are disclosed as censored instead of being silently dropped.
* Connection cost is probed separately (DNS / TCP / TLS). Stages that cannot be
measured are reported as ``not measured`` rather than inferred by subtracting
unrelated quantiles, and a failing diagnostic never discards the evaluation
report that was already collected.
* Moving a tool out of ``tools`` is a **reduction in protection**, not a pass:
unprobed cases are reported as a coverage gap alongside the scores, and a scope
that leaves a labeled population with no evaluated case fails its coverage gate.
Real responses are never written to disk; the report keeps verdicts,
probabilities, model versions and timings only.

"""Anchor: the TypeSafe guardrail's async path must not block the event loop.
``aevaluate`` is the path ``GuardrailMiddleware.awrap_tool_call`` awaits, so any
blocking call inside it stalls every other task on the loop. This anchor drives
it against a **real loopback HTTP server** — a socket-level peer, not a
``MockTransport`` — because a mock never touches ``socket.recv`` and would stay
green even if the async path were rewritten to use a synchronous client.
Teeth: ``test_sync_evaluate_on_the_loop_trips_the_gate`` runs the provider's
*sync* path against the same server from inside the loop. Blockbuster raises
``BlockingError`` there because the blocking socket calls happen with a
``deerflow`` frame on the stack. That is exactly the failure a regression
(``aevaluate`` delegating to ``evaluate``) would produce, so the green test
above cannot pass vacuously.
The provider creates its client per evaluation from ``transport_factory``; this
anchor uses the default (no factory) so the traffic runs over a real socket.
